Objective:To investigate the early clinical outcome of direct anterior approach for total hip arthroplasty(DAA)and posterolateral approach for total hip arthroplasty(PLA)for the treatment of hip osteoarthritis secondary to developmental dysplasia(Crowe type I and type II).Methods:The PLA group contained 10 patients(10 hips)and the DAA group contained 10 patients(10 hips).All the patients accepted total hip arthroplasty in Traditional Chinese Medicine of Jiangsu Province.All the operation were conducted by the same operator.All the patients were well followed up(PLA group:averange 9.3 months,DAA group:averange 8.6 months).The evaluation of two groups focued on Harris score,the dislocation rate,blood loss and the decreased levels of postoperative hemoglobin.Results:PLA group and DAA group both achieved satisfactory clinical outcomes.There were no complications such as infection and component loosening at the end of follow-up.Preoperative Harris score in two group had no significant difference.(PLA:55.7±2.75,DAA:56.1±2.81,Z=-0.306,P>0.05).Postoperative Harris score improved significantly in both group according to the follow-up.(PLA:preoperation 55.7±2.75,postoperation 74.2±4.71,P<0.05;DAA:preoperation 56.1±2.81,postoperation 83.2±4.29,P<0.05).Compared with the PLA group,DAA group had less blood loss(PLA:256.00±74.72,DAA:167.50±32.25,P<0.05),less decrease of postoperative hemoglobin(PLA:29.30±12.88,DAA:15.90 ±8.53,P<0.05)and less dislocation rate(PLA:1 dislocation,DAA:0 dislocation).The postoperative Harris score of DAA group was statisticly higher than PLA group.(PLA:74.2±4.71,DAA:83.2±4.29,P<0.05).The postoperative Harris score were higher in the DAA group which blood loss,dislocation rate and decrease of postoperative hemoglobin were less.Conclusion:Regarding the treatment of hip osteoarthritis secondary to developmental dysplasia(Crowe type Ⅰ and type Ⅱ),DAA is a minimal invasive surgery that has less blood loss,low dislocation rate,faster recovery and well-function. |
