Object:To evaluate the clinical effects and analyze the failure reason of the Cobalt-Chromium alloy-metallic Resin-Bonded Fixed Partial denture for replacing the single missing tooth.Method:Choose 26 cases who were given treatment in the department of stomatology of the First Affiliated Hospital of Dalian Medical University from 11.2014 to 12.2016,all patients were missing the anterior tooth or premolar tooth,there were 21 cases of maxillary and 5 cases of lower jaw.We didn’t do the Cobalt-Chromium alloy-metallic Resin-Bonded Fixed Partial denture for the patients whose teeth have large area restorations,or who have bruxism habit,or whose oral environment is complex and wasn’t good for cement,or who was more sensitive for caries.Carrying out the Cobalt-Chromium alloy-metallic Resin-Bonded Fixed Partial denture based on the principle of Resin-Bonded Fixed Partial denture,then cemented it with Super Bond C&B.Then follow these patients up after 1 month and the whole year.The evaluation criteria shall be in accordance with the one of the standard criteria of the USPHS and Ryge,then shall be divided into three levels(A.B.C)The clinical effects of Resin-Bonded Fixed Partial denture shall be assessed by two experienced dentists mainly on The tooth retention,Secondary caries rate,The interproximal contact,The degree of marginal coloration,The postoperative sensitivity,The condition of restoration,the marginal adaptation,The surface wear intensity,The color satisfaction of the restoration.The evaluation results for A,B level are regarded as a success,Evaluation of C level is considered as a failure.Result:After checking these 9 aspects,all of these 26 restorations have achieved A level after 1 month.After 1 year,the follow up rate is up to 96.15%with 1 loss to follow-up.The result showed that 3 cases have a little marginal coloration;1 case has the space between restoration and the surface of tooth but no mobility,1 patient was not satisfied with the color.But all these 5 cases reaching to level B;4 cases fall off;other 16 cases’ evaluation criteria and the satisfaction of patients were up to 100%.The reasons of these 4 shedding cases are secondary caries,tooth axis was not consistent,bond operation problem,occlusal high.These 4 cases had been done the cementation again after removing the factors caused failure.and the 3-month-short-term effect of these 4 restorations was perfect.We had a periodontal disease patient,the abutments besides the space had different mobility,result in the metal wing of the tooth which has less loose degree was failed,but the other metal wing was still strong.We grinded the loose metal wing and polished it.We changed the bilateral Resin-Bonded Fixed Partial denture into the cantilever Resin-Bonded Fixed Partial denture,and we didn’t regard this case as a failure.Conclusion:Cobalt-Chromium alloy-metallic Resin-Bonded Fixed Partial denture has the advantages of minor injury to dental pulp,almost no anesthesia,good maneuverability,lower prices,although it failed first time,we can do the rigidly fixed bridge or other restorations,at the same time,the short-term effect of replacing the single missing tooth with Cobalt-Chromium alloy-metallic Resin-Bonded Fixed Partial denture is well,especially for the cases who are lack of the distance between the jaws and the cases who bites tightly.It is a kind of therapy which is semipermanent and minimally invasive to generalize. |
