Background: Atopic dermatitis is a chronic inflammatory skin disease that is characterized by intense itching and recurrent eczematous lesions.Although it most often starts in infancy and affects two of ten children,it is also highly prevalent in adults.It is the leading non-fatal health burden attributable to skin diseases,inflicts a substantial psychosocial burden on patients and their relatives,and increases the risk of food allergy,asthma,allergic rhinitis,other immune-mediated inflammatory diseases,and mental health disorders.Topical corticosteroids are still the first-line therapy for acute flares,taking into account the adverse of drug reactions,so there is a strong need for safe and effective alternatives for the treatment.The development of topical immunomodulatory medications like tacrolimus has provided an alternative therapy for chronic inflammatory skin disorders.It is now recognized as a lifelong disposition with variable clinical manifestations and expressivity,in which defects of the epidermal barrier are central.Present prevention and treatment focus on restoration of epidermal barrier function,which is best achieved through the use of emollients.After all,we conducted a clinical study on the combination of two drugs.Objective To observe the clinical efficacy and safety of tacrolimus ointment and moisturizing cream in patients with atopic dermatitis(AD).Methods 68 adult patients with atopic dermatitis were randomly allocated to tacrolimus ointment and moisturizing cream group,tacrolimus ointment only group.Treatment was given twice daily to all affected areas for 3 weeks in a randomized study.Follow-up visits were scheduled at the baseline,1,2,and 3 weeks after the start of treatment.Clinical outcome parameters included the total response rate,eczema area and severity index score(EASI score),the percentage of body surface area(BSA%)affected,physician,s global evaluation(PGE),visual analog scale(VAS)pruritus score.Safety was assessed based on adverse events reported by patients or observed by the physicians.Results A total 68 with AD were enrolled in the efficacy evaluation.The respective efficacy rates of first、second and third week were 29.41%、58.82%、85.29% and 20.59%、44.12%、64.71% in patients treatment group and control group.At the end of the treatment.the physician’s global evaluation of clinical response(PGEs)at the first、second and third week were 32.35%、64.71%、88.24% and 20.59%、47.06%、70.59%,with the Eczema Area and Severity Index(EASI)score from 21.24 and 20.91 to 5.59 and 7.88,and The mean percent body surface area(BSA%)from 32.29 % and 32.82% to 9.47% and 15.11%.0f these patients,after 3 week,85.29% achieved a satisfactory outcome.35.29% achieved a complete cure.And all achieved an improvement with no exacerbation observed.The 3-week treatment also resulted in a significant decrease in the patient’s assessment of pruritus(VAS pruritus score)from 5.29 and 5.03 to 2.03 and 3.94.Transient skin burning,itching and stinging were the most common adverse events on the drug application site.Skin burning was the only sign,which occurred after tacrolimus treatment.Conclusion Treatment with tacrolimus ointment and moisturizing cream is synergistic effect and definite curative effect in treating atopic dermatitis in adult patients.And it is better than the single use of tacrolimus.With the extension of time,the treatment effect will increase.
