Objective To investigate the effect of hepassocin(HPS)in diagnosis of liver injur y caused by obstructive jaundice and the assessment of liver function in chronic liver disease.Methods Serum levels of HPS in 60 obstructive jaundice patients,68 chronic liv er disease patients and 30 healthy controls were determined by ELISA essay.Th e levels of alanine aminotransferase(ALT)、total bilrubin(TBIL)、serum albumin(A LB)、prothrombin time(PT)were tested by clinical laboratory.Chronic liver disea se patients who had no liver failure were classified by Child-Pugh classification(CPC),and patients who suffered liver failure were classified by their failure typ es.68 Chronic liver disease patients consist of 13 Child A,21 Child B,10 Chil d C,13 chronic liver failure(CLF)and 11 acute-on-chronic liver failure(ACLF).The difference of serum HPS levels between liver injury patients and healthy co ntrol group were analyzed.The correlation between HPS and ALT in obstructive jaundice patients were analyzed.The diagnostic value of HPS to liver injury resu lting from obstructive jaundice was described by receiver operating characteristicc urve.The relations of HPS with CPC and liver failure in chronic liver disease were analyzed.Results1.The serum HPS levels in obstructive jaundice and chronic liver disease patients were significantly higher than that in healthy control group(P<0.01).2.Area under curve of HPS achieves 0.938,the cut-off value of HPS as a diagn ostic index of liver injury resulting from obstructive jaundice was 141.87 μg/L.3.Statistic analysis revealing a good positive correlation between HPS and ALT i n obstructive jaundice patients(rs=0.914,P<0.01).4.Child A、B and C groups in chronic liver disease without liver failure along w ith acute-on-chronic liver failure and chronic liver failure groups were compared each two groups.Serum HPS levels gradually increased following the rise of CP C in patients without liver failure(P<0.05).Serum HPS levels in both two liver f ailure groups were significantly higher than that in each group without liver failu re(P<0.05).Serum HPS levels between ACLF and CLF groups had no significant difference(P=0.58).Conclusion1.Obstructive jaundice and chronic liver disease patients will rise their serum H PS levels to repair liver.2.Serum HPS levels can be used to diagnose liver injury in obstructive jaundice patients with higher accuracy and specificity.3.Serum HPS levels have good correlation with ALT in obstructive jaundice pati ents,and it can reflect the severity of liver injury as a specific hepatocyte prolif eration factor.4.HPS can also assist in the evaluation of liver function in patients with chroni c liver disease conbined with CPC.It is of great value to the prediction of occu rence of liver failure and the decrease of mortality and morbidity.
