Objective To assess the prognostic value of the 3rd-day-postburn neutrophil count to platelet count ratio(NPR)for severely burned patients.Methods A total of 610 severe burn patients who were admitted to the burns departments of the First Affiliated Hospital of Anhui Medical University and Shanghai Ruijin Hospital from January 2008 to December 2015 were enrolled in this study,of which the third day post-burn laboratory results of neutrophil count and platelet count were collected.Based on the median NPR of the third day after injury,the patients were divided into two groups: high NPR group(≥0.099)and low(<0.099)NPR group.Data on gender,age,inhalation injury,mechanical ventilation,early surgical treatment,and burn index were analyzed.Processing the data with chi-square,Kaplan-Meier survival analysis,and Cox regression analysis,we drew a receiver operating characteristic(ROC)curve of NPR value for 88 severe burn victims to investigate the prognostic value of the 3-day-postburn NPR.Results The high NPR group and the low NPR group showed the in-hospital survival rate of 73.5% and 91.5%,respectively,in which the difference was statistically significant(P<0.01).Cox regression analysis suggested that the NPR(HR:0.572;95%CI: 0.360-0.909)of the third day were independently associated with the prognosis for severe burns(P<0.01).The ROC curve of 88 death cases showed that the total area of the third-day NPR was 0.707(95% CI:0.650-0.764,P<0.01)and0.099 was the optimal threshold value of the third-day NPR(sensitivity: 60.2%,specificity: 72.4%).Conclusion The results suggest that the 3rd-day NPR can serve as an independent prognostic indicator for patients with severe burn.
