Objective: To study the detection of urine arsenic contect and relationship between urinary arsenic content and gestational diabetes mellitus in pregnant women and to explore other influencing factors of gestational diabetes mellitus.Methods: In order to study the arsenic content in urine samples,the method of atomic fluorescence analysis and arsenic detection in urine were established to establish the urine sample pretreatment method.A total of 3165 pregnant women were collected urine samples and questionnaires.There are 36 urine samples from pregnant women with gestational diabetes mellitus,which is the case group.And the remaining urine samples were randomly selected 36 as control group.Results:(1)Urinary arsenic detection method for 200 μL urine sample +200 μL ascorbic acid solution +200 μL 10% thiourea solution +400 μL n-octanol + 200 μL concentrated hydrochloric acid and 2800 μL ultrapure water.The recoveries at three levels were from 98.54% to 99.92% and the relative standard deviations(RSD)were from 0.6% to 2.5%.The limits of detection of the method were 0.2099 μg/L.(2)The arsenic content of 72 samples was from 26.36 μg/L to 158.89 μg/L,and the median and quartile spacing of the case group were 96.47 μg/L and 88.78-109.05 μg/L respectively.The median and quartile spacing of the control group were 42.15 μg/L and 36.34-46.98 μg/L.(3)The correlation between urine arsenic content and smoking status was obtained by correlation analysis,Spearman rank correlation coefficient rs=0.233,p<0.05.According to stratified analysis of husband smoking,husband smoking is not the main source of arsenic exposure in pregnant women.(4)The correlation between urine arsenic content and smoking status was obtained by correlation analysis,Spearman rank correlation coefficient rs=0.233,p<0.05.According to stratified analysis of husband smoking,husband smoking is not the main source of arsenic exposure in pregnant women.Conclusion:(1)This study established a direct dilution method for the determination of arsenic in urine.The accuracy and precision are satisfactory.It is suitable for the determination of arsenic in urine.(2)The content of arsenic in 72 urine samples was 26.36 μg /L-and 158.89μg /L by this method.The median concentration of urine arsenic in the case group and the control group was 96.47 μg g/L and 42.15 μg g/L respectively.(3)The content of arsenic in 72 urine samples was 26.36 μg /L-and 158.89 μg /L by this method.The median concentration of urine arsenic in the case group and the control group was 96.47 μg/L and 42.15 μg/L respectively.(4)The content of arsenic in 72 urine samples was 26.36 μg/L-and 158.89 μg/L by this method.The median concentration of urine arsenic in the case group and the control group was 96.47 μg/L and 42.15 μg/L respectively.The incidence of gestational diabetes mellitus was 1.14% in this study.Logistic regression analysis was used to investigate the relationship between urinary arsenic contents in pregnant women with gestational diabetes,the adjusted family income per capita,BMI before pregnancy,gravidity,parity,BMI at pregnancy and mother’s HBs Ag when the urine arsenic contents in pregnant women with each increase of 10 μg/L,the risk of gestational diabetes will increase 5.15 times(OR=5.15,95%CI=2.15,12.35,p<0.001).
