ObjectiveObserve the therapeutic effect of Yi Shen Jiang Tang decoction in patients with diabetic kidney disease(DKD)Stage III with deficiency of both vital energy and yin as well as variations of urine monocyte chemoattractant protein-1(MCP-1)before and after treatment.MethodA total of 120 patients with diabetic kidney disease stage Ⅲ,who met the inclusion criteria were randomly divided into the control group(n=60)and the test group(n=60).The control group received basic treatment of blood glucose,blood pressure and blood lipid control,while the test group received oral Yi Shen Jiang Tang decoction treatment with the basic treatment.The treatment duration was 90 days.Variations of indexes,including micro-albuminuria/urine creatinine(ACR),urine MCP-1,blood glucose,blood lipid and HbAlc,and TCM syndrome score in the patients were observed before and after treatment.Results1.Before the treatment,the two groups show no statistically significant difference concerning gender,age,TCM syndrome score and lab indexes(P>0.05)and show comparability.2.Before the treatment,MCP-1 levels are significantly higher in the test group and the control group than the normal control group(P<0.01).3.The test group shows decline of blood creatinine level(P<0.05)and obvious reduction of TCM syndrome score(P<0.01)after the treatment.After the treatment,there is statistically significant intergroup difference(P<0.05).HbAlc levels decline in both groups after the treatment(P<0.05);however,there is not statistically significant difference between the two groups(P>0.05).4.Reduction of ACR level appears in both the test group and the control group(P<0.05),while the improvement in the test group is superior to the control group(P<0.05).5.Both the test group and the control group show obvious reductions of MCP-1 level after the treatment,and the improvement is significantly superior in the test group than the control group(P<0.01).6.Correlation Analysis:Urine MCP-1 shows a positive correlation with ACR(P<0.01),while urine MCP-1 and ACR both show correlation with Scr and eGFR(P<0.01).7.The total effective rates of TCM syndrome improvement are 73.12%and 46.43%respectively in the test group and the control group,while the difference between the two groups are statistically significant(P<0.01).8.The total effective rates of clinical DKD improvement are 69.64%and 44.64%respectively in the test group and the control group,showing statistically significant intergroup difference(P<0.05).ConclusionYi Shen Jiang Tang decoction reduces urine MCP-1 level,delays progression of kidney function impairment,and improve clinical symptoms in patients with diabetic kidney disease(DKD)Stage III with deficiency of both qi and yin.
