Observation On Pemphigus Wound Care Effect Of United Debridement Method Matched With Infrared Radiation

Objective:This study combines autolytic debridement and conservative sharp wound debridement to make full use of advantages of both methods,then we combines united debridement along with infrared irradiation,in order to explore a new scheme that is more effective for pemphigus wound care.Materials and Methods:We selected clinically confirmed 90 PV patients from Dec 2015 to Dec 2016 in the institute of Dermatology,Chinese Academy of Medical Sciences,and used the Table of Random Number to divide the patients into three groups:A,B and C.The three groups of patients were treated with united debridement and infrared radiation therapy,combined debridement and conservative sharp wound debridement,and we counted the wound healing time,pain scores before and after intervention,and SAS and SDS anxiety and depression scores before and after the intervention,afterwards we used Students’ T Analysis and Chi-square Test to analysis the baseline,meanwhile we used one-way analysis of variance to analyze the statistically differences among the three groups.Results:1.There were no significant differences between the three groups in gender,age,education level and degree of skin lesions(P>0.05).2.Comparison of wound healing time of three care methods:The average wound healing time of patients in united debridement and infrared irradiation group were(24.3±4.72)d,and the time of united debridement group were(28.3±4.19)d,while the mean wound healing time of Group C were(31.7±5.80)d.The ANOVA analysis showed that there was significant difference in average wound healing time of patients in different groups(P<0.001,F= 14.90):The average wound healing time of patients in united debridement and infrared irradiation group was shorter than that of patients in united debridement group,and meanwhile the mean time of group B was shorter than conservative sharp wound debridement group.3.Comparison of NAS between the three groups before and after nursing intervention:There was no significant difference in pain scores between the three groups before nursing intervention(F=0.052,P=0.949),However,The NAS score in the united debridement and the infrared radiation group was significantly lower than that of the combined debridement group and the conservative sharp wound debridement group two weeks after intervention,and at the same time the score of united debridement group was lower than that of conservative sharp wound debridement group(P<0.05).4.Comparison of SAS and SDS score between three groups before and after nursing intervention:There was no significant difference in SAS and SDS scores between the three groups of patients with PV before treatment(P=0.996,F＝0.0044)&(P＝0.925,F=0.077).But once after nursing intervention,the scores of anxiety and depression of united debridement and infrared radiation group were(44.17±5.95)and(43.97±6.06),obviously better than the other two control groups(P<0.05):while the scores of united debridement group were SAS(47.70±6.29)&SDS(47.37±6.09)and the conservative sharp wound debridement group were SAS(51.00±6.28)&SDS(50.53±4.14).Conclusion:In the practice of pemphigus vulgaris wound care,combining infrared radiation with united debridement can significantly relieve pain,promote wound healing process and reduce the level of anxiety as well as depression compared with using united debridement or conservative sharp wound debridement separately.Therefore,this kind of PV wound care scheme is worthy of clinical promotion.
