ObjectiveOn the basis of evidence-based medicine,we tend to construct the questionnaire on the cognition and care needs to incontinence-associated dermatitis(IAD)of the primary caregivers of stroke patients by the Delphi method,in this way to figure out the IAD cognition status,the needs to IAD prevention and care in the primary caregivers of stroke patients,and to lay the foundation for providing targeted and effective health education and guidance to the primary caregivers of stroke patients.MethodPart I: Based on the review,analysis and arrangement of related literatures and existing clinical work experience,we initially formed a questionnaire on the cognition and care needs to IAD of the primary caregivers of stroke patients,with group discussions and modification,we primarily formed the first edition of questionnaire for expert review.Then we selected 15 specialists and nursing managers in incontinence care to make additions or amendments to the dimensions and contents of questionnaire with two rounds of the Delphi consultation,then,we discussed and revised again and again based on the feedbacks of experts,finally we in experts,and ultimately we construct the questionnaire on the cognition and care needs to IAD of the primary caregivers of stroke patients,and we performed the test on the reliability and validity of questionnaire.Part II: we conducted a survey on 150 primary caregivers of stroke patients with incontinence in the Su Bei People's Hospital by tool of constructed questionnaire,all data are analyzed and processed in SPSS19.0 statistical software.ResultPart I: we selected 15 experts with 100% experts actively coefficient and 0.88 expert authority level in two rounds of consultation,finally,we formed the questionnaire including two parts: the cognition part consist of three dimensions(knowledge,skills,prognosis)with 10 items,and the part of care needs includes three dimensions(knowledge,skills,social support).Part II: 1.The total questionnaire score on the cognition part was 22.92 ± 6.12,in primary caregivers of stroke patients with incontinence with a score range of 13 to 42 points,and the caregivers scored highest in the IAD's "prognosis" item(2.84 ± 0.91),while lowest in the IAD theoretical knowledge(2.24 ± 0.72).2.The total questionnaire score on the part of care needs was59.11 ± 9.53 with a score range of 43 to 75 points,and the caregivers scored highest in the item of IAD relevant skills(4.02 ± 0.72),while lowest in the social support needs(3.85 ± 0.80).3.Significant cognitive and care needs difference among caregivers with different characteristics:significant cognitive difference among caregivers with different age,gender and education level were found(P <0.05),specifically,those caregivers with age<40 years,female and college and undergraduate education level showed more IAD recognition level(P <0.05).Meanwhile,significant care needs difference among caregivers with different education level and family income were found(P <0.05),the caregivers with college and undergraduate education level and higher family income(3000RMB/month)showed more care needs on IAD(P <0.05).4.A positive correlation between caregivers' recognition level and care needs on IAD(r = 0.354,P = 0.000)was found.A negative correlation between caregivers' age and the recognition level on IAD was found,and a positive correlation between caregivers' age,education level and recognition level of on IAD was found was found(P <0.05).A negative correlation between caregivers' age,gender and care needs on IAD was found,and a positive correlation between caregivers' education level,family income and care needs on IAD was found.(P <0.05).5.The multiple factors regression analysis showed that the difference between caregivers culture degree and cognitive dependent variable was found,(P < 0.05),the difference between caregivers culture degree and care demand was found,(P < 0.05).ConclusionPart I: The questionnaire on the cognition and care needs to IAD of the primary caregivers of stroke patients has shown good reliability and validity,which can be adopted for clinical investigations.Part II: 1.The primary caregivers of stroke patients appear to have a knowledge shortage to IAD,wherein know most about the prognosis of IAD,but least about the IAD prevention and care,more education to caregivers on this issues should be addressed.2.The primary caregivers of stroke patients show a number of needs on the IAD care,with less needs on social support and more needs on IAD prevention and care,multi-methods are needed to satisfy the care needs of the primary caregivers of stroke patients.3.Adequate attention to the cognitive level and care needs of caregiver in the course of treatment and care are warranted,besides,we should take into account the caregiver' gender,age,education level and family income to provide targeted health education,at the same time,thus to raise IAD prevention and care awareness of the primary caregivers of stroke patients,and improve the life qualityof stroke patients.
