[Objective] To explore the the feasibility and effectiveness of the application of cognitive intervention based on the transtheoretical model (TTM) in type 2 diabetic patients with mild cognitive impairment. By evaluating the patient’s stage of cognitive behavior, giving the phase matching cognitive intervention, to reduce the degree of cognitive dysfunction% to improve the ability of daily life activities and diabetes self-management, eventually to establish the good behaviors for the health of patients, reduce complications and improve the quality of life of purpose.[Methods] It was prospective randomized controlled study, select 120 type 2 diabetic patients with mild cognitive impairment who admitted in Geriatric Medicine department of Nanjing Drum Tower Hospital from January 2014 to June 2015, Computer generated random number, the singular number were control group(n=60) and the dual number were experimental group (n=60).Control group was given conventional cognitive intervention, including memory ability training, space obstacle training, logical ability, operation ability and training, daily living skills training and so on. Intervention group was given the cognitive intervention according to TTM, by evaluating the patient’s stage of cognitive behavior, giving the phase matching cognitive intervention.Before intervention,1 month, 3 months,6 months after intervention,will evaluate the behavior change(the evaluation table of the stages of behavior change), cognitive function(MMSE) and daily life ability (Barther Index), diabetes self-management ability (DSCS),and Blood sugar control target (2hPBG、Hb)of two groups.[Results]1. Compared with the general demographic information of the two group:including age, sex, occupation, marital status, educational level, family income, source of medical costs and living way, caregivers, duration and frequency of occurrence hypoglycemia, showed no significant difference (P>0.05)2.There is no statistically significant differences between two groups in stages of behavior change before intervention and 1 months after intervention (P> 0.05),with significant differences between two groups at 3 months and 6 months after intervention (P<0.05).3. Compared with both groups of cognitive function and daily life activity, There is no statistically significant differences in scores of MMSE and Barther Index before intervention and 1 months after intervention (P>0.05),with significant differences at 3 months and 6 months after intervention (P<0.05).4. Compared with both groups of self-management ability and blood sugar control index, There is no statistically significant differences in score of SDSCA,2hPBG and Hb before intervention and 1 months after intervention (P> 0.05),with significant differences at 3 months and 6 months after intervention (P<0.05).[Conclusion] Cognitive intervention based on the TTM is effective for the behavior change of patients who has type 2 diabetes mellitus with mild cognitive impairment, also can increases participation、reduce dropout rates and improve the cognitive function、 self-management ability and blood glucose control level of patients who has type 2 diabetes mellitus with mild cognitive impairment.lt is appropriate to use in chinic.
