Clinical Effect Of Single-port Traction Video-assisted Thoracoscopy Surgery In The Treatment Of Primary Spontaneous Pneumothorax

Objective:To analysis the clinical effect of single-port traction,two-port and single-port video-assisted thoracoscopic surgery in the treatment of primary spontaneous pneumothorax.Methods:A total of 226 patients with primary spontaneous pneumothorax underwent thoracoscopic surgery from May 2014 to October 2016 were divided into two groups(166 cases)and single port group(20 cases)and single port traction group(40 cases).The average operative time,anesthesia time,bleeding volume,postoperative drainage rate,tube time,length of hospital stay,pain score,hospitalization cost and recurrence rate of each groups were analyzed.To analysis whether there is statistical difference among two port method,single port and single port traction method.Results:In this study,204 cases were male,22 females,male and female ratio was 10:1;147 cases of Han nationality,68 cases of Korean,the other 11 cases,the ratio was 13:6:1;the first episode of 210 cases,multiple episodes of 16 cases,the ratio was 14:1;mild pain in 118 cases,moderate 100 cases,severe 8 cases,the ratio was 15:12:1;There was one case appeared as recurrence during the follow-up dates of the two-port method group.Comparison of operation time between two-port traction method and two-port method(42.8±16.2 vs 37.3±12.5),the difference was statistically significant(p<0.05),The difference was not statistically significant with single-port method(47.4 ± 11.9);Comparison of bleeding volume between single-port traction method and two-port method(20.26±9.44 vs 38.51±13.34),the difference was statistically significant(p<0.05);Comparison of bleeding volume between single-port traction method and single-port method(20.26±9.44 vs 15.52±1.57),the difference was statistically significant(p<0.05).Comparison of pain score between single-port traction method and two-port method(3.18± 1.15 vs 3.900±1.45),the difference was statistically significant(p<0.05),The difference was not statistically significant with single-port method(2.95±1.13);The difference was not statistically significant in anesthesia time,postoperative drainage,tube time,length of hospital stay and hospitalization cost.Conclusion:1.Single-port traction method VATS can reach the same result with two-port method and single-port method in the treatment of primary spontaneous pneumothorax.2.Single-port traction method VATS in the treatment of primary spontaneous pneumothorax is safe,reliable and light postoperative pain advantages,As the technology continues to mature can shorten the operation time.It is worthy of clinical promotion and application in the treatment of spontaneous pneumothorax.Traction assisted method can be used for local resection of lung tumor,lung biopsy and any other thoracoscopic surgery.
