| Objective:With the rapid growth of economy and the increasingly competitive society,the proportion of older pregnant women in the overall pregnant women grows higher.A variety of risk factorsleading to an increased risk of birth defects in elder pregnant women.This study selected 77 cases of birth defects in elderly pregnancy,compared the final pregnancy outcome and prenatal ultrasonic examination results,analyzed the distribution feature of fetal deformity disease and reasons of misdiagnosis,evaluated clinical value and its influencing factors of ultrasound on birth defects in the elderly pregnant women,to provide references for ultrasound screening in birth defects.Methods:77 elderly pregnant women with birth defects were selected from from March 2013 to June 2015 in Linyi people’s hospital as cases of study group.Voluson-730.Voluson-E8 and Philips IU22 color Doppler ultrasonic diagnostic instrument produced by the American GE company were used for antenatal examination.Data was analyzed by SPSS 20.0..Results:1.This study included 3274 cases of elderly pregnancy,of which 77 cases were confirmed as birth defects.The birth defect rate was 2.35%,the single deformity were 54 cases,accounting for 70.13%and multiple deformity were 23 cases,accounting for 29.87%.2.Of these fetuses and newborns,23.37%(18/77)had cardiovascular malformation,were classified as major;22.07%(17/77)had multiple system malformation;14.28%(11/77)had central nervous system abnormalities;12.98%(10/77)had symptom complex;7.79%(6/77)had splits;5.19%(4/77)had urinary and genitourinary system malformations;5.19%(4/77)had musculoskeletal systemmalformations;3.89%(3/77)had pulmonary malformations;3.89%(3/77)had digestive system malformations;1.29%(1/77)had eye and ear deformities.The proportion of syndrome complex growed higher.3.Sixty seven abnormalities were detected prenatally,yielding a diagnostic accordance rate for prenatal detection of fetal abnormalities of 87.01%(67/77),and a rate of miss diagnosis of 12.99%(10/77).4.Overall,the missed diagnosis rate of maxillofacial deformities and skeletal system deformities were higher,prevalence 66.67%and 33.33%respectively;while the missed diagnosis rate of central nervous system abnormalities deformity,cardiovascular system and digestive system malformation were lower.5.The ultrasound diagnosis coincidence rate has statistical difference among the following five factors,including prenatal pregnancy tested times,gestational age,the number of births,working experience of doctors.There was no statistical difference between single deformity and multiple deformity.Conclusion:1.The women of maternal advanced age have higher incidence rates of fetal defects,cardiovascular malformation were classified as major;followed by multiple system abnormalities,nervous system abnormalities,symptom complex,splits,urinary and genitourinary system malformations.2.Major influence factors on coincidence rate of ultrasonic diagnosis of fetal birth defects includes prenatal pregnancy tested times,gestational age,the number of births,working experience of doctors. |
