Observation On Management Of Hypertension Follow-up In Community Of Songling Town

Objective1.To understand the status of hypertension in Songling Town of Wujiang District.2.To understand the follow-up management of hypertensive patients in community of Songling Town,Wujiang District.3.To explore the effect of the follow-up management of hypertension and its existing problems and improvement measures.MethodFollow up management of hypertension patients in a community of the song,after the community health education for patients and according to blood pressure using different antihypertensive drugs reasonably,allowing patients to change bad living habits,reasonable diet,reduce the risk factors of hypertension,the blood pressure was controlled effectively,the blood pressure control rate has been effectively improved.A random sampling method was used to sample 2013,2014,respectively,2015 patients with hypertension in community extraction,comparing 3 years through 3 years observation data of blood pressure,hypertension patients follow-up management classification of blood pressure controlResult1,2013 to 2015,the community in the town of Songling female patients with hypertension than male patients,the proportion of male and female patients remained relatively stable.2,through the management of hypertension,different gender,different age groups of patients with hypertension in the management of data before and after the existence of significant differences in blood pressure values have been effectively controlled.3,The proportion of men and women in different grades of management is different.Different levels of management of hypertension in patients with hypertension through the management of grading,blood pressure before and after the management of significant differences in blood pressure values have been effectively controlled.4,2013 ~ 2015,the high rate of hypertension evaluation index increased year by year,Songling community hypertension patients through the management of grade follow-up,blood pressure value has been effectively controlled,the effect increased year by year.ConclusionThrough the follow-up management of hypertension classification,the blood pressure of hypertensive patients in Songling town in 2013,2014 and 2015 was effectively controlled,and the good rate increased year by year.Community hypertensive follow-up management of hypertensive patients with blood pressure control effect is obvious,it is worth further promotion.
