Study On Efficiency For Medical Resources Allocation Of Ningxia County Public Hospital

Background:Country Public hospitals are very vital for the construction of primary medical and health system.With the implementation of new health care reform and financial investment gradually increase,which we know that optimizing the distribution of medical and health resources and rational allocation is the focus of the reform.In this context,whether the rational allocation of County Public Hospitals and the maximization of output to come true or not is the important issues to focus on.Therefore,evaluating the efficiency of Count-level Public Hospitals are important.Objective:This study chooses 27 County Public Hospitals.On the basis of the efficiency of medical resources allocation related theories and in combination with the practical situation of medical resources allocation and social economic development of Ningxia,this paper selects Factor analysis and Coefficient of Variation to build the model which evaluates the efficiency of medical resources allocation and to measure the efficiency of medical resources allocation on 27 County Public Hospitals by Data Envelopment Analysis(DEA),so as to provide refer basis on optimizing allocation of medical resources,finding the existing problems and shortage,proposing improvement measures.Methods:Combining with the descriptive analysis and SAS statistical software,this paper analyzes the medical resources allocation of Country-level Public Hospitals in Ningxia.The Factor analysis and Coefficient of Variation are used to construct the model which evaluates the efficiency of medical resources allocation.The study adopts Data Envelopment Analysis to measure the efficiency of medical resources allocation in 27 County Public Hospitals.Results:1.The situation of medical resource allocation of Country Public Hospitals in Ningxia:aspects of total resources,Country Public Hospitals indicated a growing trend of medical resources in Ningxia from 2009 to 2014.During 2009 to 2014,the amount of workers,the amount of bed,fiscal subsidies income and fixed assets increased annually by 14.30%、32.37%、22.53% and 5.28% in County People’s Hospital.County Hospital of traditional Chinese Medicine increased by the speed of 10.14%、18.23%、43.42% and 7.36%.2.Combining with the result of Factor analysis and Coefficient of Variation,this paper ultimately defines 4 input and 3 output indicatrix which evaluates the efficiency of DEA medical resource allocation,including bed number,employee number,fiscal subsidies income and fixed assets,business income,the number of diagnose and treat patient,the number of discharge patient.3.In the horizontal study,the DEA evaluation of efficiency of Country-level Public Hospitals in Ningxia:only 11 hospitals had completely positive effective,accounting for 40.74%;while 16(59.26%) hospitals had relatively ineffective.During 2009 and 2014,the number of technical efficiency,pure technical efficiency and scale efficiency of DEA effective changed little in 27 County Public Hospital of Ningxia.The number of technial effciency of DEA effective is 11,the proportion is 40.74%.The number of pure technical efficiency of DEA effective is 19,the proportion is 70.37%.The number of scale efficiency of DEA effective is 11,the proportion is 40.74%.From 2009 to 2014,the number of DMU which on the state of increased return to scale is 6,the number of DMU on the state of decreased return to scale is 10.From the view of input and output slack variable value,the main reasons for the low efficiency of the investment is medical services insufficient.4.In the longitudinal study,the configuration results of 2009 and 2010 in the six years of 2009-2014 present relatively ineffective.In 2009,the scale return to decreased;while the scale return to increased in 2010.From 2011 to 2014,DEA is effective and the state of constant scale changed little.5.The total factor productivity of Country Public hospitals of Ningxia Prefecture decreased 14.5% during 2009 and 2014,which caused by the progress of technical efficiency(14.2%).Yearly analysis shows,The total factor productivity increased at the period of 2011-2012 and 2012-2013.The total factor productivity increased depended on technical efficiency at the period of 2010-2012,and which increased depended on technic improving at the period of 2012-2013.Under the condition of the total factor productivity,there are 12 DMUs’ total factor produtivity increased and there are 10 DMUs’ technical efficiency increased and there are 16 DMUs’ technic improving increased from 2009 to 2014.In addition to 2010 and 2011,the change of total factor productivity index has a certain growth in addition to 2010 and 2011,especially the growth rate of 2013 is largest.Suggestions:1.Strengthening the human resources construction to promote the improvements of technology,technical efficiency and scale efficiency.2.The government should play the leading role in guiding the scale development of Country Public Hospitals rationally.Optimal allocation of medical resources distribute the regional medical resources planning rationally.3.A reasonable allocation of medical resources and improve the efficiency of resource use is necessary.
