Analysis On Metabolic Syndrome And Health Management Strategies In Tianjin Physical Examination Population

Objective: Through a comprehensive survey of the lifestyle of Tianjin physical examination population, aiming to analyze the prevalence of metabolic syndrome(MS) and screen the main influencing factors.Combined with the concept of health management and China's national conditions, provide policy recommendations for the prevention and management of MS in the future.Methods: 10980 physical examination subjects were investigated by questionnaire, physical measurement and laboratory tests. Using chi-square test, canonical correlation analysis(CCA), and logistic regression analysis, to study the differences of prevalence of MS under different behaviors, screen the influencing factors, and analyze the correlation between independent variables and the MS components. Results:(1) The prevalence of MS is 22.76%, and the prevalence in males is 31.98% and in females is 9.67%.(2) Under different independent variables, such as age, BMI, cultural level, the types of work, family history(CVD, diabetes, high BP), smoking, drinking, physical activity, sitting, TV time, bacon, fruit, pickled vegetables, dairy products and coffee, males had the significant differences of prevalence of MS(P<0.05). Under the basic characteristics(except for family history) and life behavior factors(except for sleep and eat bacon, vegetables, pickled vegetables, bean products) females had the significant differences(P<0.05).(3) Through Logistic regression analysis, it was found that often drink coffee had the strongest protective effect on MS, the following were university and above culture level, plenty of exercise, milk products, drink occasionally, often eating fruits, eating breakfast every day, often eating white meat, and eggs(OR<1, P<0.05). While the harm to MS were obese, overweight, male, ?45 years old, TV time ?3 h/d, often eating bacon, smoking, family history of diabetes, family history of hypertension, and often eating pickled vegetables(OR>1, P<0.05).(4)Through CCA, it was found that the typical correlation coefficients of the basic characteristics, life behavior and MS components are 0.564 and 0.764, which can determine the overall change of MS component in 34.62% and 62.12%. BMI, gender,and age played the major role among the basic characteristics, while waist circumference(WC), diastolic BP, and HDL-C played the major role in relative components of MS. Drinking, eating fruits, dairy products, pickles, and smoking played the major role among the life behaviors, while WC, systolic BP, and TG played the major role in relative components of MS.(5) The population attributable risk percentages were calculated when six living behaviors were combined. There were 10 people meeting the behavior combination of “reasonable diet, plenty of exercise, non-smoking, occasionally/non-drinking, TV time <3h/d, eating breakfast every day”. Compared to those who unmet standard, it could reduce 68.73% population attributable risk. Among 45 and above years old females, there were 4 people meet the standard, and no one had MS.Conclusion: The prevalence of MS in Tianjin physical examination population was high, but few people couldhave a healthy lifestyle. Due to the lack of health management, the government and people should arouse enough caution, set up the advanced idea of health management, design the system and construct the organization system of the health management committee. Meanwhile, the domestic service institutions should be improved, for instance, the function and the medical model of medical centers should be extended and updated, resident information system needs to be improved, and the wearable devices should be promoted and applied. In addition, with the theory and method of traditional Chinese medicine, the joint promotion of communities, enterprises and families should achieve full coverage of health management topromote the healthy development of the whole people.
