Objective: This study have been taken saliva and gingival crevicular fluid to d-etcet the level of C-reactive protein factor(CRP)and blood sample have also bee-ncollected to observe the concentration of interleukin-17(IL-17)and interleukin-23(I L-23)bu enzyme linked immunosorbent assay(ELISA)compared with patients only with periodontitis and healthy controls.Methods: 1.The research object:according to the relation between periodontiti-s and RA and diagnostic criteria in healthy subjects,selected Patients with rheumato-id arthritis(R)of 28 people,patients with chronic periodontitis(P)32 people,rheum-atoid arthritis patients with periodontitis with 22(R + A)and healthy controls(H)18 people.2.The basic data and periodontal examination: each participant molars were measured and recorded the clinical indicators: probe bleeding index(bleeding on probing,BOP),calculus dissuade index,(CI),probing the depth,(PD),attachment loss,(A L).3.Enzyme-linked immunosorbent method were used to detect serum IL-17 all subjects,the content of TNF alpha,IL-23,and the content of CRP in all subjects of stimulative saliva.4:Statistical analysis: comparing two groups of mean using single factor analysis of variance and the least significant method,the influence of different periodontal status of RA onset using multivariate Logistic regression analysis,using t he Spearman correlation analysis between the two factors.With double side inspecti on method for all data statistics.Results: 1.The level of TNF-α,IL-17,IL-23 in serum form the patients of group R,P and R+P were higher than group H,the differences were statistically significant(P<0.05).The level of TNF-α,IL-17,IL-23 in serum form the patients of group R+P were higher than group P and RA,and the level of TNF-α,IL-17,IL-23 in serum for the patients of group p and RA have no obvious difference,no statistical significanc-e.2.Clinical periodontal index,according to the comparison of R+P,P group BOP positive rate,PD and AL is higher than the RA group(p<0.05);The above three grou-ps of BOP positive rate and AL were higher than in group H(p<0.05);R+P grou-p of PD and AL is higher than that of P group,but no statistical significance(P>0.05)3.R+P,RA group and P group of patients with gingival sulcus of CRP level was significantly higher than that of group H,R+P and P group patients saliva and CRP levels were significantly higher in gingival sulcus liquid in RA patients(p<0.05),it was statistically significant(p<0.05).And correlation analysis showed that the RA gr-oup,the R+P group and P group of CRP levels in patients with gingival sulcus liq-uid with periodontal probing depth,attachment loss levels were positively correlated(p<0.05).Conclusion:1.TNF-α,IL-17,IL-23 May play an important role in the relationship between periodontal disease and RA,cause periodontitis inflammatory factors can als-o affect the rheumatoid arthritis.2.By clinical examination showed that periodontal probe bleeding index,the depth of the probe,attachment loss heaviest damage in pati-ents with periodontitis with rheumatoid,pure periodontitis patients,the health group o-f times,it may also mean RA may aggravate development degree of periodontitis le-sions.3.In gingival sulcus fluid examination found that the highest CRP level of pa tients with periodontitis,significantly higher than the healthy group and RA,suggests periodontal inflammation makes oral local higher CRP,through blood circulation and increase the risk of suffering from RA. |
