Background and aims: She ethnic population is one of the minorities in China, and there is rare evidence on the essential hypertension susceptibility genes in this population, especially in persons who have metabolic syndrome. Hence, we conducted this study to analysis the relationship of CYP17A1 gene with essential hypertension and famine exposure in She population with metabolic syndrome in Fujian Province, and the interaction between famine exposure and CYP17A1 gene on hypertension.Methods: All participants in this study were unrelated She population in Ningde city of Fujian province. According ATPIII diagnostic criteria on metabolic syndrome, 698 subjects with metabolic syndrome were included, and then these subjects divided into two cohorts based on hypertension diagnostic criteria, including 486 subjects with hypertension and 212 with normal blood pressure. Physical measurements were performed and clinical biochemistry data were collected. Genomic DNA was extracted from peripheral blood, and then genotyping was performed with ABI 7500 system based on Taqman probe technology. Data analyses was conducted with Windows SPSS 17 statistical software package(SPSS, Chicago, Illinois, USA).Results:(1) The frequencies of CC genotype, CT genotype and TT genotype in CYP17A1 gene(rs11191548) were 5.2%, 40.7% and 54.1%, and the frequency of the risk allele T is 74.4%. The frequencies of TT genotype and T allele in CYP17A1(rs11191548) were both higher in hypertension group than normal hypertension group(P<0.05).Compared C allele carriers, participants who carrying T allele was found to be 1.495 times(OR=1.495,95% CI: 1.129, 1.98, χ2=7.92,P=0.005)more risk for hypertension group.(2)After adjusting for age, sex and body mass index(BMI), we found TT genotype carriers of CYP17A1(rs1191548) had higher level of systolic blood pressure(SBP) compared with CC genotype carriers.(3)After adjusting for age, sex, body mass index(BMI), HDL-C,LDL-C,TG,TC,FBG,2h PG,smoker and alcohol use, logistic analysis revealed that compared with CC genotype carriers, TT genotype carriers of CYP17A1(rs11191548) to be more risk of hypertension(OR=2.714, 95%CI: 1.222, 6.028). In the additive model, the risk allele T carriers found to be 1.6 times(OR=1.6, 95%CI: 1.174,2.81, P=0.003) more risk of hypertension than not carriers; in the dominant model, TT and CT genotype carriers found to be 2.232 times(OR=2.232,95%CI:1.027,4.853, P=0.043) more risk than CC genotype carriers. In the recessive model, TT genotype carriers were founded to be 1.667 time(OR=1.667,95%CI:1.143,2.43, P=0.008)more risk than CC+CT genotypes carriers.(4) We defined subjects born during October 1, 1959 and December 31, 1961 as famine exposed cohorts, and subjects born during October 1, 1962 and December 31, 1965 as non-famine exposed cohorts. After adjusting for sex, body mass index(BMI), HDL-C,LDL-C,TG,TC,FBG,2h PG,smoker and alcohol use, linear regression reveals that famine exposure had relation with SBP(β=8.329, P=0.006),but had no significant relation with DBP(β=1.731, P=0.295).(5)Linear regression revealed that after adjust confounding factors, TT genotype carriers and CT+CC genotype carriers who had famine exposure had relation with SBP, but no cohorts had relation with DBP.Conclusion: Polymorphism in CYP17A1 was significantly associated with essential hypertension in She population with metabolic syndrome, and T allele was considered as the risk allele of hypertension. Furthermore, famine exposure during fetal period had relationship with SBP, and famine exposure had interaction effect with CYP17A1(rs11191548)on hypertension.
