The Summary Of Professor Hou Yufen’s Academic Thoughts And Clinical Experience And The Clinical Research On The Treatment Of ASO With Qi-defic-iency And Blood-stasis Syndrome

Objective:To research and summarize the academic thoughts and clinical experience of Prof. Hou yufen in treating peripheral vascular diseases, especially in arteriosclerosis obliterans(ASO) with qi-deficiency and blood-stasis syndrome, and to explore the therapeutic mechanism for it.Methods:(1)Studying and analyzing the data from collecting medical records, referring tutor’s and graduated students’ papers and manuscripts, and consulting the tutor, etc.(2)80 ASO patients were divided into two groups, extra washing therapy group and non-washing group. We observed the change law of all patients by recording and comparing the syndrome integral, ankle brachial index(ABI), blood flow perfusion and transcutaneous oxygen pressure in pre and post treatment.Results: The results were classified into “Overview”, “Academic Thought”, “Clinical Experience” and “Clinical Research”.(1)“Overview” mainly introduced tutor’s biography and medical practice experience. Professor Hou yufen is a famous surgeon of TCM,one of fourth and fifth veteran teran doctors of TCM, and a guide teacher of academic experience. She has been in the fields of clinical and teaching for about 40 years, especially been good at the diagnosis and treatment of vascular surgery diseases by integrated traditional Chinese and western medicine, and she has rich clinical experience and unique academic opinions.(2)“Academic Thought” was composed of several aspects.(1) Replenishing qi to invigorate spleen, adjusting and reinforcing liver and kidney. Prof. Hou holds that peripheral vascular surgery diseases are closely related to spleen, kidney, and liver. The spleen is the origin of the acquired constitution, the kidney is the origin of congenital constitution, the liver governing conveyance and dispersion, and the three are independent. So Prof. Hou emphasizes the dysfunction of the three organs in the peripheral vascular diseases, and she proposes that in the treatment, we should pay attention to adjust and reinforce liver and kidney, and dredge the meridians, qi and blood.(2) Strengthening healthy qi to eliminate pathogens and considering the superficial and radical aspects. Prof. Hou thinks that the essence of diseases concludes the sthenia pathogenic factors and asthenia healthy Qi, and in the diagnosis, we should observe both the local lesions and the whole constitution. In the treatment of peripheral vascular surgery diseases, Prof. Hou grasps the main pathogenesis—stasis, meanwhile differentiates the cold, heat, asthenia, sthenia, yin, and yang. She emphasizes a clear discrimination of superficial and radical aspects is essential to establish the therapy.(3) Converging the traditional and modern medicine, differentiating syndrome and treating disease. Prof. Hou masters not only the differentiation syndrome and treatment in traditional Chinese medicine, but also the anatomy, physiology, pathology, diagnosis, treatment and scientific research work in modern medicine. She thinks that although the traditional medicine and the modern medicine are two different medical theory system formed on different history background, they have the same therapeutic aim, and each has advantages and disadvantages. Prof. Hou emphasizes that we should combine the disease and syndrome differentiation, macroscopic and microscopic syndrome differentiation, differentiation syndrome and treatment and western medicine.(4) Paying attention to external therapy and being good at proved recipe. Prof. Hou pays attention to external therapies including fumigation and washing, external application, lace and debridement and so on to achieve the optimal efficacy. In the application, she emphasizes the combination of macroscopic syndrome and microscopic syndrome differentiation. Prof. Hou holds that the folk prescriptions or proved recipes are always mistake in the composition or indication because of they are transmitted from mouth to mouth. So we should discrimination the proved recipe to ensure the drugs can match the disease and syndrome.(3)“Clinical Experience” focuses on the tutor’s treatment therapy and typical medical cases to diabetic limb arterial obliteration and chronic leg ulcer.(4)The results of “Clinical Research” show that the treatment can cause the decline of syndrome scores and the raise of ABI. The obvious curative rate was 83.75% and the security index kept normal. Although the curative effect was different between extra washing group and no-washing group, the change had no statistical significance. The baseline value, the value after heating and the increasing rate of blood perfusion, and the transcutaneous oxygen pressure were increased after the treatment. The increasing rate of blood perfusion and the transcutaneous oxygen pressure in extra washing group were higher than no-washing group, and the difference was significant(P<0.05). Therefore, the extra washing can improve the microcirculation index more effective than no-washing.Conclusion: We summarized the academic thoughts, therapeutic therapy and drug administering characteristics of Prof. Hou in the treatment of peripheral vascular diseases by arranging the data systematically. By analyzing the treatment of diabetic limb arterial obliteration and chronic leg ulcer, we explored the law of combination of integral and local syndrome differentiation, syndrome and disease differentiation, macroscopic and microcosmic syndrome differentiation, and summarized the characteristics of paying attention to strengthen healthy qi, applying simultaneously internal and external therapy and consideration of liver, spleen and kidney. By the clinical research, we found that the therapeutic therapy of Prof. Hou was effective to the ASO patients with qi-deficiency and blood-stasis syndrome and could improve the symptoms and examinational results. Based on this, if the patients were treated by external washing therapy, the blood flow perfusion and transcutaneous oxygen pressure will be improved better.
