Objective: To investigate idiopathic membranous nephropathy expression(IMN) and clinical significance of peripheral blood B cells in FcγRⅡB.Methods: The percentage of CD19+B cells, the percentage of B cells subpopulation and the mean fluorescence intensity of FcγRⅡB on peripheral blood B cells from idiopathic membranous nephropathy patients and healthy controls was examined by flow cytometry;According to the proteinuria levels,the IMN patients were divided into the massive proteinuria group(proteinuria quantity≥3.5g/d) and the non-massive proteinuria group(proteinuria quantity<3.5g/d); To analysis the correlation between B cells FcγRⅡB expression levels and clinical parameters in patients.Collecting experimental date and then doing the statistics and data analysis by applying SPSS 21.0 software,set the inspection level α=0.05.Results:1、The percentage of CD19+B cells was significantly higher in IMN patients [(14.51 ± 3.26)%, n=24] when compared to normal control group[(8.47 ± 1.69)%, n= 24](t =-8.053, p <0.05),and the percentage ofCD19+B cells in IMN patients was positive correlation with 24-hour urinary protein quantity(r=0.658,P<0.001).2、The percentage of CD19+CD27+ memory B cells subpopulation was significantly lower in IMN patients[(12.86 ± 1.42)%, n = 24] when compared to normal control group[(31.61 ± 8.72)%, n = 24](t = 10.398,p <0.05); the percentage of CD19+CD27- naive B cells subpopulation was significantly higher in IMN patients[(85.89 ± 1.50)%, n = 24] when compared to normal control group[(67.38±8.72)%,n=24](t=-10.247,p<0.05).3、The mean fluorescence intensity of FcγRⅡB on CD19+CD27+memory B cells from the the massive proteinuria group of IMN patients[(61.4 ± 8.7), n = 17] was significantly decreased when compared to the non-massive proteinuria group of IMN patients[(94.2 ± 13.0), n = 7,p <0.05] and normal control group[(110.2 ± 13.2), n = 24, p <0.05],and the MFI of FcγRⅡB on CD19+CD27+ memory B cells from the the non-massive proteinuria group lower than the control group(p<0.05).The mean fluorescence intensity of FcγRⅡB on CD19 low CD27high plasma B cells from the the massive proteinuria group of IMN patients[(34.6 ± 7.8), n = 17] was significantly decreased when compared to the non-massive proteinuria group of IMN patients[(46.6 ± 4.8), n = 7,p <0.05] and normal control group[(53.7 ± 14.4), n = 24, p <0.05],But the mean fluorescence intensity of FcγRⅡB on CD19 low CD27high plasma cells from the the non-massive proteinuria group compared to the normal group was not statistically significant(p>0.05).The difference of Themean fluorescence intensity of FcγRⅡB on CD19+CD27- naive B cells from the the massive proteinuria group of IMN patients[(72.03 ± 7.7), n =17],the non-massive proteinuria group of IMN patients[(73.7 ± 1.2), n =7, p> 0.05] and normal control group[(77.7 ± 12.9), n = 24, p> 0.05]was not statistically significant.4、Further studies showed that the mean fluorescence intensity of FcγRⅡB on CD19+CD27+ memory B cells(r=-0.646,p<0.05) of IMN patients and CD19 low CD27high plasma cells(r =-0.416, p <0.05) of IMN patients were negative correlation with the 24-hour urinary protein quantity. The mean fluorescence intensity of FcγRⅡB on CD19+CD27+memory B cells of IMN patients was positive correlation with total serum Ig G level(r = 0.424, p <0.05).Conclusion:The percentage of CD19+B cells was significantly higher in IMN patients when compared to normal control group,and the percentage of CD19+B cells in IMN patients was positive correlation with 24-hour urinary protein quantity; The mean fluorescence intensity of FcγRⅡB on CD19+CD27+ memory B cells from was IMN patients was significantly decreased when compared to the normal control group,and The mean fluorescence intensity of FcγRⅡB on CD19+CD27+ memory B cells were negative correlation with the 24-hour urinary protein quantity, and The mean fluorescence intensity of FcγRⅡB on CD19+CD27+ memory B cells of IMN patients was positive correlation with total serum Ig G level; The mean fluorescence intensity of FcγRⅡB on CD19 low CD27high plasma Bcells from was IMN patients was significantly decreased when compared to the normal control group,and The mean fluorescence intensity of FcγRⅡB on CD19 low CD27high plasma B cells were negative correlation with the 24-hour urinary protein quantity.The abnormal expression of FcγRⅡB may play a important role in the immune pathogenesis of idiopathic membranous nephropathy patients.
