Section I The comparative study of high intensity focused ultrasound and radiofrequency ablation in the treatment of hepatocellular carcinoma(diameter<5cm)Objective To compare the efficacy of two kinds of local ablation therapy in the treatment of hepatocellular carcinoma(diameter<5cm).Methods Collection the hepatocellular carcinoma(less than 5cm in diameter) 122 cases with minimally invasive treatment of the First Affiliated Hospital of Zhengzhou University from 2009.09 to 2014.09, which by high intensity focused ultrasound(HIFU) in the treatment of 57 cases(HIFU group), radiofrequency ablation(RFA) in the treatment of 65 cases(RFA group), and two groups of patients with clinical data and follow-up results were statistically analyzed.Results HIFU and RFA for hepatocellular carcinoma(diameter<5cm) in 1 month tumor volume ablation rate(P = 0.409), after treatment the level of alpha-fetal protein(AFP) decreased(P = 0.475) was not statistically significant; HIFU group 1 year, 3 year overall survival rate were 87.8%, 59.4%, and RFA group 1 year, 3 year overall survival rate was 93.6%, 60.4%, 1 year survival rates of RFA group was slightly better than the HIFU group, and the two groups of patients overall survival rate without significant statistical difference(log rank test p = 0.558).Conclusions Two kinds of local ablation therapy for hepatocellular carcinoma(diameter<5cm)were effective; two kinds of local ablation therapy after 1 years survival rate of RFA group was slightly better than the HIFU group, there was no statistically significant difference between the 2 groups.Section II The associated complications analysis and treatment of High Intensity Focused Ultrasound in the treatment of primary hepatocellular carcinomaObjective To investigate the complications of High Intensity Focused Ultrasound(HIFU) in the treatment of primary hepatocellular carcinoma, looked forward to improving the safety of the treatment of HIFU.Methods Retrospectively analyzed the clinical data of 165 patients with primary liver cancer treated by High Intensity Focused Ultrasound in the First Affiliated Hospital of Zheng Zhou University from December 2009 to September 2014, to summarise the complications of this therapeutic method and analysis the related treatment measures.Results HIFU treatment of primary liver cancer common complications included the treatment of skin soft tissue injury(118/ 165,71.5%),a transient injury of liver function(83 / 165,50.3%),postoperative fever(81/ 165,49.1%),self-limiting pleural effusion(42 / 165,25.5%) and postoperative pain(33 / 165,20.0%); The severe complications included rib fractures(2 cases), organ perforation injury(1 case), Cancer rupture hemorrhage(1 case)and Skin II(5 cases), III(1 case) degree burn; Serious consequences were not caused by the complications which were actively treated.Conclusions The treatment of primary liver cancer by High Intensity Focused Ultrasound has less serious complications and high safety.
