| Objective:To investigate the clinical efficacy of western medicine lipoic acid combined with foot bath of traditional Chinese medicine treatment of diabetic peripheral neuropathy who is dialectical belongs to the type of blood stasis,and discussion the possible mechanism and its therapeutic advantage.Methods:Eighty cases with DPN who is dialectical belongs to the type of blood stasis from June 2013 to August 2015 in the traditional Chinese medicine department and endocrinology department of Wuhan Third Hospital were used in this study.The patients were randomly divided into the control group(n=40) and the therapy group(n=40). Both groups were given general treatment,the control group was give n Alpha lipoic acid injection,one time a day,the therapy group was given foot bath of traditional Chinese medicine based on the control group,one time a day,and both groups were treated for 14 days.The clinical effect,the indexes of fasting blood glucose(FBG),postpra ndial 2 hours blood glucose,fasting C peptide,postprandial C-pepti de,blood lipid,bilateral posterior tibial artery and dorsalis pedi s artery blood pressure,ABI,vibration perception thresholds,TCSS scores and clinical symptoms were observed and compared in two groupsResults:1.Laboratory examination:Firstly,there is no signifca nt difference in FBG,2h postprandial blood glucose,fasting C peptid e and postprandial 2h C peptide after treatment in two groups(P>0.05).Secondly,blood lipid,there is no significant difference in four lipid indicators in the control group after treatment(P>0.05) who without use the drugs of lipid-lowering, but the TC and LDL-L of therapy group was declined signifcantly after treatment(P<0.05); the TC and LDL-L were decreased significantly after treatment(P <0.05) in the control who had use the drugs of lipid-lowering,but the four lipid indicators of the therapy group were declined signifi cantly after treatment(P<0.05).2. clinical examination: Firstly,t he brachial artery systolic blood pressure,foot dorsal artery systo lic blood pressure and the tibial artery systolic blood pressure of the two group was declined significantly after treatment(P<0.05),but the fall of therapy group was significantly better than the control group.Secondly, there is no significan difference in ABI value was observed in control groups after treatment(P>0.05),while the therapy group decreased significantly(P<0.05).In addition,the number of who’s ABI returned to normal in therapy group was obvious ly higher than that of control group.Thirdly,the vibration percepti on threshold:there is no significant difference in control group after treatment compared with before treatment(P>0.05),but there is a great significant difference in therapy group(P<0.01).3. Clinical efficacy analysis:firstly,there is no significant differe nce in DPN clinical grade distribution was observed in control group after treatment(P>0.05),but significant difference was observed in the therapy group(P<0.05).Secondly,the TCSS scores and the single main symptoms scores in both groups were deeply declined after trea tment(P<0.05),but the decline of control group is not so obvious as the treatment group.Thirdly,the clinical efficacy for the therap y group and control group was 92.5% and 70.0% respectively,and the efficiency of therapy group was obviously higher than that of control group(P<0.05).And in the course of the treatment,both of the two groups had no obvious adverse reactions.Conclusion:Fristly,Lipoic acid combined with traditional Chinese medicine therapy can reduce the blood lipid(TC,LDL-L),blood pressu re,ABI,and so on,it had significantly improve the blood circulation of limb.Secondly,lipoic acid combined with foot bath of traditional Chinese medicine for treating DPN who blongs to blood stasis is significantly,and obviously improved their quality of life.What’s more,the foot bath of traditional Chinese medicine is simple,econom ical,safe and effective,is worth to be application on clinical. |
