The Study On The TCM Syndrome Differentiation And Medicine Usage Regulation Of Breast Cancer

ObjectiveBased on 122 cases of breast cancer hospitalized patients age, syndrome type, frequency of symptoms, cancer cell metastasis, drug usage regulation were analyzed, the study focuses on the regularity of drugs distribution with the frequency greater than 10% and provides an effective basis for clinical medication and prevention.MethodsAccording to the diagnosis standard of breast cancer, collecting a total of 122 patients with breast cancer were in the oncology department of the First Affiliated Hospital of Guangzhou University of Chinese Medicine. And according to the diagnostic standard of breast cancer diagnosis and treatment in TCM, the patients were treated with traditional Chinese medicine, and the establishment of Excel database. The database was finished by statal2.0 and SPSS17.0 statistical software with deleting the redundant variables and similar variables in the database, and finally the descriptive statistical analysis and factor analysis on the general information, clinical symptoms and the regularity of drug use.ResultsIn this study, the age of onset was between 31 and 76 years old. There are 76 patients between 31 to 56 years old,34 patients 56 to 66 years old,11 patients 66 to 76 years,1 patient in 31 years old, were 62.30%, 27.87%,9.02%,0.82%.According to clinical type distribution, the type which the proportion is more than 5%are:Syndrome of deficiency of the spleen and stomach(33 cases,27.05%), Syndrome of stagnation of liver qi and phlegm coagulation(30 cases,24.59%), Syndrome of liver stagnation and spleen deficiency (20 cases,16.39%), Syndrome of phlegm and blood stasis (8 cases, 6.56%).79.51% of the patients presented the clinical symptoms of the spirit of fair, and the symptoms is higher than 15%, mainly is the malaise, poor physical strength, poor appetite and sleep, abnormal stools, redness and swelling of the chest mass, chest pain, upper limb and shoulder radiating pain, aches in the waist and whole body, bitter taste and dry mouth. Tongue and pulse mainly performance for white fur, thin white fur, pale tongue, dark red tongue, dark tongue, thready pulse, wiry pulse.The results of the disease transfer situation are as follows:the most of breast cancer patients with cancer cells of the organ metastasis, metastasis to bone up to 33 cases, accounting for 27.05%, which cancer cells transferred to the liver, lymph nodes are as high as 15% or more. Followed by metastasis to the liver with 26 cases, accounting for 21.31%. Metastasis to the lymph nodes with 20 cases, accounting for 16.39%. Transferred to the lung patients in this study is relatively at least, only 17 cases, accounting for 13.93%.Clinical treatment of breast cancer patients in TCM are mainly about nourishing qi, smoothing the liver, activating blood, nourishing blood, promoting qi, and reducing phlegm. The syndrome of deficiency of the spleen and stomach mainly treated with chinese medicine filling the spleen qi, compatibility of promoting qi, circulating blood and reducing phlegm drugs. The amount of white peony root, tuckahoe, cooked Codonopsis pilosula, Banxia, Amomum villosum, Atractylodes, licorice is relatively large, each accounting for more than 10% of the deficiency of the spleen and stomach syndrome frequency of administration. The syndrome of stagnation of liver qi and phlegm coagulation mainly are used to drugs having the function of promoting qi reducing phlegm, of which the frequency accounted for more than 10% of the drug have Banxia, Amomum villosum, Fructus aurantii, Bulbus Fritillariae thunbergii, white paeony root, tangerine peel, white atractylodes rhizome, cooked Codonopsis pilosula, liquorice, poria, crude Rhizoma Curcumae. Liver stagnation and spleen deficiency syndrome in the effectiveness of the medication with emolliating liver promoting qi and nourishing the spleen qi. The frequency of administration of more than 10% of the drug followed by white peony root, tuckahoe, Radix Fici Hirtae, cooked dangshen, white atractylodes rhizome, astragalus root, coix seed, yam, tangerine peel, amomum fruit, liquorice, plum, citrus aurantium. The syndrome of phlegm and blood stasis are mainly used the blood-activating and stasis-dispelling drugs, and the compatibility of promoting qi, nourishing qi of the medicine. The medication frequency higher than 10% of the drug have processed semen persicae, pinellia, Bulbus Fritillariae thunbergii, Radix Fici Hirtae, Fructus aurantii, amomum fruit, astragalus, Panax, dried tangerine peel, mountain arrowhead, tuckahoe, radix curcumae, Radix Paeoniae Alba, cooked Radix Codonopsis, Atractylodes, licorice, Diospyros lotus, coix seed, chinese yam.Using factor analysis between drugs first compatibility of nourishing qi and promoting qi, its main function is:strengthening the spleen and replenishing qi, dispersing the stagnated liver qi; the second compatibility to invigorate the circulation of blood and qi, and its main function is promoting blood circulation and removing blood stasis, invigorating spleen and soothing liver qi; the third compatibility for soothing the liver and clearing heat and removing phlegm and its efficacy indications:heat detoxification carbuncle of soothing the liver and regulating qi and resolving phlegm; the fourth of compatibility of spleen qi, soothing liver and nourishing kidney, nourishing the blood and conciliate resistance. The main symptoms are:nourishing spleen qi and blood, harmonic, emolliating liver, nourishing yin; the fifth of compatibility of inducing diuresis and excreting dampness, strengthening spleen and promoting digestion, its efficacy indications:strengthening spleen, disinhibit water, permeability and promoting digestion.ConclusionThe results of this study showed that:The patients with breast cancer are mostly women at the age of 31-56 years old. They can still take care daily life by themselves and the majority show clinical symptoms of mental fair. The main clinical syndromes are the deficiency of spleen and stomach syndrome, liver qi stagnation and phlegm stagnation syndrome, liver stagnation and spleen deficiency syndrome, phlegm and blood stasis syndrome. And most breast cancer patients have cancer cells in the organ metastasis. The patients metastasis to the bone are the most, followed by the liver, lymph, metastasis to the lung of patients with relatively minimal. According to the medicine usage regulation of this disease, the medicine are mainly about nourishing qi, smoothing the liver, activating blood, nourishing blood, promoting qi, and reducing phlegm. The syndrome of deficiency of the spleen and stomach mainly treated with chinese medicine filling the spleen qi, compatibility of promoting qi, circulating blood and reducing phlegm drugs, including white peony, Poria, Radix Codonopsis, cooked pinellia, Atractylodes, licorice, Amomum relatively large amount; the syndrome of stagnation of liver qi and phlegm coagulation mainly are used to drugs having the function of promoting qi reducing phlegm, Banxia, Amomum, Fructus aurantii, Radix Paeoniae Alba, Fritillaria thunbergii, tangerine peel, Atractylodes, Codonopsis, Poria, licorice, cooked coarse zedoary majority; liver stagnation and spleen deficiency syndrome in the effectiveness of the medication with emolliating liver promoting qi and nourishing the spleen qi, the majority of white peony root, tuckahoe, Radix Codonopsis, Atractylodes, Radix Fici simplicissimae cooked, Huang Qi, coix seed, Chinese yam, tangerine peel, amomum fruit, licorice, Fructus aurantii, Heizao; the syndrome of phlegm and blood stasis are mainly used the blood-activating and stasis-dispelling drugs, and the compatibility of promoting qi, nourishing qi of the medicine, mostly processed walnuts, pinellia, Fritillaria, peach, Fructus aurantii, Amomum, Huang Qi, three seven, tangerine peel, Poria, radix curcumae, shancigu, Radix Paeoniae Alba, Radix Codonopsis, Atractylodes, licorice, ripe prune, coix seed, chinese yam. Factor analysis showed that 12 common factors in the main effect for the* strengthening spleen", "emolliating liver", "activating blood", "reducing phlegm", "promoting qi", mainly related to the spleen deficiency, stagnation of the liver qi, blood stasis, qi stagnation, phlegm dampness. The "strengthening spleen" frequency is 5 times, "promoting qi" is 4 times, "emolliating liver" frequency is 3 times, "activating blood" appeared twice, "reducing phlegm" appeared twice.
