Objective To retrospective analysis and compare the clinical outcomes of unilateral versus bilateral kyphoplasty for osteoporotic compression fracture.Methods Having selected 223 cases of senile patients,from December 2012 to June 2015,with osteoporotic vertebral compression fractures in Wuhan No.5 hospital,male 56 cases,female 167 cases,aged from 61 to 88, with an average age of 68.3 years.Thoracic section 124 segments, lumbar section 156 segments, a total of 280 segments. According to the position of puncture, they have been divided into two groups, Unilateral group of 115 cases, vertebral body 154 segments,bilateral group of 108 cases,vertebral body 126 segments.Unilateral group is in the way of the unilateral pedicle puncture, with bone cement injection of vertebral body center line is beautiful; Double side groups are double side wear the bone cement injection,. After treatment for 3 days, compared two groups of patients with operation time, visual analog pain score changes before and after operation, vertebral body height recovery rate, vertebral body of the protruding after Cobb Angle to improve and postoperative bone cement leakage rate to assess the patient’s clinical curative effect.Results:unilateral puncture group and bilateral puncture operation time(30.84±1.95 vs41.28±1.94min)comparative differences are statistically significant differences(t=18.94,P < 0.001). Unilateral and bilateral group average vertebral body height recovery rate( 15.18±0.45% vs 15.84%±0.34%) comparative differences had no statistical significance(t=0.96,P > 0.05); Unilateral and bilateral group visual analog pain score(1.52 ± 0.65 vs 1.76 ± 0.66)is no statistical difference(t=1.295,P >0.05); Unilateral and bilateral group recovery of Cobb Angle(13.64 °± 1.66 vs 13.24°± 1.42), is no statistical difference(t=0.91,P >0.05); Unilateral leak rate is far lower than the double side group(χ2 =4.00,5.84% vs 12.69%), there was a significant statistical difference(P < 0.05).Depend on 0~48 months(average 16 months) of follow-up visit,we find it that 1 cases of all appear the symptom of nerve root,such as flank pain,especially which is worse in rainy weather,and others did not develop complications such as infection, pulmonary embolism and adjacent vertebral body fracture. Conclusion:unilateral and bilateral pedicle percutaneous vertebral plasty in the treatment of osteoporotic vertebral compression fractures can achieve satisfactory analgesic effect, and the former has advantages of shorter operation time, low incidence of postoperative complications.
