| Objective:The purpose of this study is that To analyse the complications of radiofrequency ablation in patients with benign thyroid nodules,to investigate the risk factors which influenced complication of radioferquency ablation and the effectiveness of the preventive measures.Methods:1.A retrospective analysis was made on the patients who have 1017 nodules get radiofrequency ablation between April 2010 to October 2015 at our department.In accordance with the Alder grading evaluation internal blood flow of the nodule,according to the flow distribution evaluation nodules peripheral blood flow.Based on the location of nodules to grade the risk of injury of recurrent laryngeal nerve.Record the clinical puncture pathway.Statistical radiofrequency ablation treatment of complications of the total number of cases,and Statistical the incidence of hemorrhage in different blood flow signals in and around nodules,the incidence of hoarseness in different laryngeal recurrent nerve risk grading.The incidence of bleeding on different puncture path.Generalise the relationship between different risk factors and complications.2.Statistical the total complications of the 596 nodules that accept radiofrequency ablation therapy and no prevention measures.Alder grading between-group,the incidence of hemorrhage between the isthmus group and the lateral neck group were analyzed,According to the the laryngeal recurrent nerve injury risk to analyse the incidence of complications in between groups different voice hoarse.Comparative differences between groups.3.596 nodules that get radiofrequency ablation between April 2010 to August 2014 at our department as no prevent group,475 nodules that get radiofrequency ablation between September 2014 to October 2015 at our department as prevent group.Records the incidence of complications and statistical analysis in the two groups.Comparing differences between two groups.Results:1.1071 benign thyroid nodules are completely ablation,no transfer to open surgery.14 cases of total complications(1.31%),10 cases were hoarseness(3 cases of perithyroidal hematoma,3 cases of supcapsular hematoma,and 4 cases of intranodular hematoma),3 cases of hoarseness,1 case of nodule rupture.hemorrhage associated with puncture path,blood flow signals in and around the nodules.Laryngeal recurrent nerve injury associated with the position of thyroid nodule.2.13 cases of complications in 596 benign thyroid nodules that get radiofrequency ablation,3 cases of hematoma,3 cases of hoarseness,1 case of nodule rupture.This study indicates that distributions of blood flow signals in and around the nodules,puncture paths were significantly different(P<0.05).The incidence of hemorrhage about nodules Alder level 3 significantly higher than odules Alder level 0 and Alder level 1,the incidence of hemorrhage about peripheral blood flow level 1.the incidence of hemorrhage about lateral neck group higher than isthmus group.the incidence of recurrent laryngeal nerve injury about level 4 higher than other level.3.Differences in the incidence of complications was statistically significant between non-prevention group and prevention group(P<0.05),The incidence of complications of non-prevention group was significantly higher than prevention group.Conclusion:1.blood flow signals in and around the nodules,and puncture path are risk factors of hemorrhage.Nodule location is a risk factor of recurrent laryngeal nerve.2.Hemorrhage prone to internal and peripheral blood-rich nodules,through the side of the neck puncture easily lead to Hemorrhage,Hoarseness prone to nodules located at the low part of thyroid gland.3.To understand the risk factors of complications,and take appropriate preventive measures are helpful to lower the incidence of related complications. |
