Analysis Of Case Reports About Clinical Pharmaceutical Care And Establishment Of Writing Quality Evaluation Standards

Objective: To retrieve,analyze and evaluate the case reports about clinical pharmaceutical care,in order to investigate the clinical practice of clinical pharmacists in China and provide reference for further development of clinical pharmaceutical care and cultivation of clinical pharmacists.To establish the writing quality evaluation standards and detailed scoring rules of case reports about clinical pharmaceutical care based on the existing problems in writing,in order to provide advice and reference for clinical pharmacists and promote the normative,scientific and integrative of these documents.Methods: With pharmaceutical care,clinical pharmacy,clinical pharmacist as the key words,literature retrieval was made through the Chinese Journal Full-text Database in China National Knowledge Infrastructure(CNKI),Weipu China Science and Technology Journal Database,Wangfang data and PubMed.Case reports about clinical pharmaceutical care published from 2000 to 2014 were collected.The following items were statistically analyzed: the relationship between numbers of documents and publishing time,the education background and professional title of the first author,the geographical distribution of the first working unit of the first author,the hospital grade of the first author’s working unit,the departments of clinical pharmacist performing clinical pharmaceutical care,drug categories of clinical pharmaceutical care focusing on,main contents of clinical pharmaceutical care provided by clinical pharmacists,the corporation of clinical pharmacists with medical personnel in other departments.Based on the existing problems in writing,The CARE guidelines,the published paper “How to write a patient case report”(Cohen H.Am J Health-Syst Pharm,2006,63:1888-1892)and the characteristics of clinical pharmacy,(1)First,to establish the preliminary evaluation standards,and the case reports about clinical pharmaceutical care published from 2000 to 2014 were evaluated by these standards.(2)Based on the discovered problems,the evaluation standards were further refined,and the writing quality evaluation standards and detailed grading rules of case reports about clinical pharmaceutical care were established.(3)The selected documents with relatively complete contents were evaluated by established standards.Results: Totally 1348 documents were collected.The number of the documents increased annually.The first authors mainly came from eastern and middle area of China.The grade A tertiary hospitals accounted for 85.46%.The departments of clinical pharmacist performing clinical pharmaceutical care were exceeded 30,and clinical pharmacists mainly provide pharmaceutical care for patients in medical department such as Department of Gastroenterology,Department of Cardiology and Department of Neurology.The anti-infective drugs,drugs acting on digestive system,hormonal and related drugs,drugs mainly acting on cardiovascular system were the drugs clinical pharmacists mainly focusing on.The drug varieties selecting,discovery,monitoring and handling of adverse drug reactions,advice for drug application and dosing,patient education were the main forms of pharmaceutical care of clinical pharmacists providing.There were many problems in writing of case reports about clinical pharmaceutical care,the documents with relatively complete contents were only 310(23.0%)and these documents were evaluated by the standards established,and the highest score were 94,the scores of 90.65%(281 documents)of the literature were 70-89.The top 3 missing parts of these literatures were experience summary of the case and advices for diagnose and treatment,patients’ previous history and related medical literature analysis.The documents co-signed by clinical pharmacists and medical personnel in other departments were only 152(11.28% of all the documents).Conclusion: The clinical pharmaceutical care practice developed rapidly in recent years,and clinical pharmacists’ work were recognized by physicians,nurses and patients to a certain extent.But this work mainly developed in grade A hospitals and the working developments are not balanced in different area of China.The communication and corporation between clinical pharmacists and medical personnel in other departments need to be strengthened.According to the existing problems in writing of case reports about clinical pharmaceutical care,the writing quality evaluation standards and detailed grading rules were established in this study,and it could provide some reference for clinical pharmacists to write case reports about clinical pharmaceutical care and be used to find superior authors and high-quality articles.
