| Objective: The purpose of this study was to evaluate the clinical practical value of ultrasound elastography and magnetic resonance imaging in the diagnosis of breast lumps.To analyzed the advantages and limitations of the two methods.To provided more valuable reference information for clinical diagnosis and treatment of choice.Method:To confirmed by surgery pathology of 110 cases of female patients with breast lumps,a total of 146 lesions.To exceed by high frequency colour,ultrasound elastography and magnetic resonance imaging befor operation.Ultrasonography and magnetic resonance imaging time interval of not more than a week.To analyzed and compared the sensitivity,specificity and accuracy in diagnosing of breast malignant lumps.To analyszed the related statistion.Result:1.The sensitivity,specificity and accuracy of color doppler flow imaging(CDFI)according to Adler blood flow classification in diagnosing breast malignant lumps were 85.53%(65/76),84.29%(59/70),84.93%(124/146).2.The sensitivity,specificity and accuracy of spectrum doppler(SD)according to the resistance index(RI)in diagnosing breast malignant lumps were 84.21%(64/76),85.71%(60/70),84.93%(124/146).3.The sensitivity,specificity and accuracy of ultrasound elastography according to bao-ming luo puts forward the improved method of five points in diagnosing breast malignant lumps were 94.74%(72/76),88.57%(62/70),91.78%(134/146).4.The sensitivity,specificity and accuracy of magnetic resonance imaging in diagnosing breast malignant lumps were 96.05%(73/76),88.57%(62/70),92.47%(135/146).5.To compared the sensitivity and accuracy of the joint utrasound with the single method in diagnosing malignant breast lumps haved statistical significance(P < 0.05).There was no statistically significant difference in the diagnostic methods of magnetic resonance imaging(P > 0.05).There was a statistically significant difference in the microcalcification of high frequency ultrasound and the magnetic resonance imaging(P < 0.05).6.The biggest diameter of the breast lumps measured by magnetic resonance imaging and ultrasound elastography were positively correlated with the pathological measurement value.The magnetic resonance measurement value is close to the pathological measurement value,and the ultrasound elastography is the second.Conclusion:1.To showed by magnetic resonance imaging in multifocal and multicentric lesions were superior to ultrasound.The biggest diameter of breast malignant lumps measured by magnetic resonance imaging is closer to the pathological measurement value.More realistic response of breast cancer pathological staging of preoperative evaluation and intended to keep breast surgery treatment choose to help.But the magnetic resonance on the tiny calcification was poor,easy to misdiagnosis and missed diagnosis of only a single feature of micro calcification of the malignant lumps.2.Ultrasound elastography can be used as the primary diagnostic imaging examination methods of breast lumps,and it has an important role in the diagnosis and follow-up of breast lumps.Ultrasound examination for microcalcifications display is more sensitive than MRI,the diagnostic accuracy of ultrasound elasticity imaging of morphological features not typical,less blood supply,calcification of the malignant tumor was higher than that of the magnetic resonance,of magnetic resonance imaging misdiagnosis of breast intraductal carcinoma play key role to compensate.At the same time,ultrasound was a good assistant tool for breast biopsy,but the penetration of ultrasound imaging was low,which showed that the depth of the breast was poor.3.Ultrasound elastography,and magnetic resonance imaging can complement each other,mutual reference in clinical practice,because with their respective strengths and weaknesses. |
