with the highest morbidity and mortality around the world.The morbidity rate of lung cancer is still rising every year.The clinical symptoms of lung cancer patients in early stage were not obvious.Most of the patients were in the middle-advanced stage when diagnosed as lung cancer,the five-year survival rate of whom is less than 5%.At present,the major treatment of lung cancer is still surgery combining with chemotherapy and radiotherapy.The main influence factors of lung cancer patients’ survival time lies in the surgery effects.Whether metastasis or recurrence appears after surgery is especially important.Many kinds of serum tumor markers related to lung cancer have been found.It is reported that they played an important role in the prognostic analysis and the effect evaluation of lung cancer.ObjectiveTo explore the value of dynamic monitoring serum CEA,CA19-9,CA125 and CA153 level in perioperative lung cancer patients.Methods58 random clinical data of lung cancer patients in our hospital from May 2014 to May 2015 were analyzed.These 58 lung cancer patients were set as lung cancer group,including 38 males and 20 females,aged between 42~78 years old(mean age 53.7±6.8 years old).All of the subjects above were diagnosed as lung cancer by bronchoscopy and biopsy,including early stage(Ⅰ period)lung cancer 18 cases,medium stage(Ⅱ period)lung cancer 33 cases,Ⅲ period lung cancer 7 cases;According to the histopathology,the subjects were divided into five groups: squamous carcinoma 29 cases,adenocarcinoma 19 cases,adenosquamous carcinoma 6 cases,adenoid cystic carcinoma 2 cases,acinic cell carcinoma 2 cases;According to the clinical course,the subjects were divided into five groups: preoperative group,1 day after operation group,1 week after operation group,1 month after operation group and 3 months after operation group.According to the differentiation degree,the subjects were divided into three groups: well-differentiation group 6 cases,medium-differentiation group 31 cases and low-differentiation group 21 cases.50 healthy cases were randomly selected as control group.The test results were normal,including 28 males,22 females,aged between 40~66 years old(mean age 51.2±5.6 years old).There was no significant difference in age and gender ratio among lung cancer group and control group.Chemiluminescence immunoassay(CLIA)was employed to determinate the serum CEA,CA19-9,CA125,CA153 levels of lung cancer patients before surgery,1 day after operation,1 week after operation,1 month after operation and 3 months after operation.Then compare the data of lung cancer group with that of control group.ResultsThe preoperative serum CEA,CA19-9,CA125,CA153 levels of lung cancer patients were significantly higher than the control group,the difference was statistically significant(P<0.05);the serum CEA,CA19-9,CA125 and CA153 levels of lung cancer patients in 1 day after surgery group,1 week after surgery group,1 month after surgery group and 3 months after surgery group were significantly lower than that of the preoperative group,the difference was statistically significant(P<0.05);the average serum CEA,CA19-9,CA125,CA153 level of lung cancer patients in 1 month after surgery group were higher than that of 3 months after surgery group,but the difference was not statistically significant(P>0.05).The differences between serum CEA,CA19-9,CA125,CA153 levels of lung cancer patients in 1 month after surgery group,3 months after surgery group and the control group were not statistically significant(P>0.05);there was no obvious correlation between the preoperative serum CA19-9 level of lung cancer patients and clinical stage;the preoperative serum CEA,CA125,CA153 levels of lung cancer patients were positively correlated with clinical stage.The sensitivity of single detection of serum CEA,CA19-9,CA125,CA153 level to diagnose lung cancer was 45.8%,26.5%,40.74%,10.4%,respectively,and the specificity of which was 92.5%,90.42%,82.13%,97.8%,respectively.The sensitivity of combined detection of those four tumor markers was 93.06%,and the specificity of which was 71.4%.Results showed that the joint detection was better than single detection to diagnose lung cancer.ConclusionMonitoring perioperative serum CEA,CA19-9,CA125 and CA153 levels in lung cancer is valuable for assessment of surgery effect.1.Dynamic monitoring of perioperative serum CEA,CA19-9,CA125 and CA153 levels of lung cancer patients can be used as clinical monitoring indicators for surgery effect;2.The combined detection of CEA,CA19-9,CA125 and CA153 is better than the single assay to be used for auxiliary diagnosis and prognosis of lung cancer. | |
