| Objective With the acceleration of the population aging,China has entered an aging society.The elderly has become the main population of chronic diseases and medicine consumption,thus health care and medication safety for the elderly have become one of the most significant social concerns.health of the elderly in poor areas has always been an important part of China’s old-age care.However,the chronic disease prevalence rate of the elderly in poor areas has increased year by year recently,and medication safety problems have also increased in the meanwhile.Therefore,this paper aims to explore the influencing factors of medication safety for older adults with chronic diseases in poor areas,in order to improve the safety of drug use in the elderly.MethodsThe 1690 elderly in the poor areas of Liaoning Province with chronic diseases who were surveyed by self-designed questionnaire which contains the general information scale and medication use circumstances scale were selected by the multi-stage random sampling.Firstly SPSS17.0 software was used to test the reliability and validity of the questionnaire for the knowledge,attitude,medication adherence of medication safety,the results show the Cronbach α=0.85,KMO=0.873,this questionnaire can be used for this survey.The general situation,health and drug use were described by frequency and percentage.Medication safety factors of the elderly with chronic diseases in poor areas were analyzed by using exploratory factor analysis and Logistic regression analysis.ResultsThe 17 influencing factors of medication safety for the elderly with chronic diseases in poor areas were selected from 19 original variables with the single factor logistic regression analysis(P<0.05).The variables were analyzed with exploratory factor analysis and 3 common factors of medication safety for the elderly with chronic diseases in poor areas were extracted: F1(financial situation),F2(medication adherence),F3(knowledge of medication safety)and F4(attitude for medication safety).The model of medication safety for the elderly with chronic diseases in poor areas was established.The regression coefficient of the model was evaluated and the result showed that P <0.05,which revealed that the model of regression coefficient was at a high level.The model fitting was evaluated by Hosmer and Lemeshow test and the result showed that P>0.05,which revealed that the model fitting was ideal.Therefore,knowledge of medication safety,attitude for medication safety,medication adherence and financial situation are the influencing factors of medication safety for the elderly with chronic diseases in poor areas.ConclusionIt was revealed by Logistic regression analysis that knowledge of medication safety,attitude for medication safety,medication adherence and financial situation are the influencing factors of medication safety for the elderly with chronic diseases in poor areas.Therefore,we should actively carry out the publicity and guidance about reasonable medication use to the elderly in poor areas,thus to improve the medication safety and self-care consciousness of the elderly and to avoid the occurrence of adverse medication reactions and adverse medication events caused by irrational medication use.In order to develop the medication safety and self-health care awareness,we should focus on adverse drug reaction and adverse drug events that result from irrational drug use.We should also avoid medical resources waste and improve the development of safe,effective and economic use of medication among the elderly in order to ensure their life quality.At the same time,constantly improve the health care system and policies in poor areas in order to solve the problem of poor elderly medical treatment in poor areas. |
