BackgroundHepatitis B virus infection is a worldwide epidemic,although in recent years the scientist has made significant progress in antiviral therapy,but the status of infection is still not optimistic.According to the World Health Organization,about 20 million people worldwide have been infected with HBV,of which 350 million people develop chronic hepatitis B virus infection,each year about one million people die from liver failure,cirrhosis,primary hepatocellular carcinoma of HBV infection [1].HBV infection is still one important public health issue which endangering the health of the masses.HBsAg is a viral envelope protein that HBV-S gene used the HBVcccDNA as the template to encode,within HBV cccDNA or intrahepatic HBVDNA good correlation exists,its quantitative detection is the new tools to monitor and predict the respond of anti-HBV treatment.HBsAg was found by Blumberg in 1965,since then a long period of time,the enzyme-linked immunosorbent assay(ELISA)method was the mainly method which was used for the diagnosis of hepatitis B virus infection.Previous study has considered that there was a good correlation between HBsAg and peripheral blood HBV-DNA/cccDNA,which is a embodiment of template library inside the liver,but the clinical significance of HBsAg quantitative detection of awareness was limited by the detection method.In recent years,with the progress and development of HBsAg quantitative detection technology,together with the gradually quantitative HBsAg and related studies,the clinical value of quantitative HBsAg caught the increasing attention,but the clinical significance and correlation studies on the quantitative detection of serum HBsAg still remain controversial.ObjectiveTo get the comprehensive understanding of the clinical value of quantitative detection of HBsAg and its influencing factors.To build the foundation for further study of comprehensive scientific significance of HBsAg or its clinical application.MethodsBy reviewing the relevant literature which related to the field of serum HBsAg quantitative detection of serum HBsAg to review the characteristic of HBsAg changes in different natural history of Chronic HBV infection,among different infection status and antiviral process,to summarize HBsAg correlation with many factors such as HBVDNA,HBVcccDNA,liver function,age,genotype and gene mutation.Results and conclusions:Quantitative HBsAg showed a regular dynamic changes in the natural course of chronic HBV infection and anti-viral treatment process.Monitoring of serum HBsAg provides an important indicator for clinicians to determine the timing to start antiretroviral therapy and anti-virus policy,and its dynamic changes in the baseline levels and its decline in anti-viral treatment process also have some predict for stable long-term efficacy.The synthesis and secretion of HBsAg influenced by so many factors such as genotype,HBV variants,age,gender,race,the initial infection time,detection,treatment strategies,and so on that the specific variation is unknown,and the correlation between HBVDNA there is controversy.Serum HBsAg showed dynamic changes in the natural course of HBV infection.There is a great value in prediction of spontaneous HBeAg and HBsAg clearance,distinguishing active and inactive HBs Ag carriers.However,in recent years,the value of HBsAg in differentiate inactive HBsAg carriers and HBeAg(-)CHB has been questioned.The changes of HBsAg quantitative values in the anti-virus has great significances in monitoring the state of the illness and evaluatting the curative effect,but a baseline lever of serum HBsAg that could forecast antiviral treatment response is not clear,the best forecast point in time and the best prediction cutoffs of HBsAg can not be determined.
