| Objective: To analysis the relationship between the expression of IL-4,CD163, VEGF and clinical significance in breast infiltrating ductal carcinoma.Method: 76 cases of surgical treatment which were diagnosed as breast infiltrating ductal carcinoma were collected and prescreened from the Fourth Hospital of Hebei Medical University during January 2007 to December 2010.According to the results of immunohistochemistry and follow-up data, the correlation of expression of CD163+ TAM, IL-4 and VEGF,clinicopathological features and prognosis were analyzed respectively and reciprocally.Result: 1 Patients generally information. The age of 76 cases patients with breast infiltrating ductal carcinoma were 28-77 years. The median age was 50 years( < 50 years, 32 cases; ≥50 years, 44 cases); The average diameter of tumor was 2.08cm( < 2cm, 29 cases; ≥2cm, 47 cases).Histopathological classification grade Ⅰ was 18 cases. Grade Ⅱ was 38 cases.Grade Ⅲ was 20 cases. The number of clinical stage I was 16 cases. Clinical stage Ⅱwas 36 cases. Clinical stage Ⅲ was 22 cases. Clinical stage Ⅳ was 2cases. Breast cancer molecular classification Luminal A was 17 cases.Luminal B was 44 cases. HER2-type was 8 cases. Basal-like was 7 cases. The number of lymphatic metastasis were 31 while no lymphatic metastasis were45. The patients with vascular tumor bolt were 28 cases while 48 cases without vascular tumor bolt.2 In breast infiltrating ductal carcinoma, the infiltrarion density of CD163+ tumor associated macrophage in tumor stroma was 12% and the median number was 17/HPF. High density was 46 cases and low density was30 cases. The infiltrarion density of CD163+ tumor associated macrophagesurround the tumor stroma was 44% and the median number was 38/HPF; The difference was statistically significant(P<0.05). The high expression level of IL-4 in breast infiltrating ductal carcinoma was 48 cases. Low expression were28 cases. The high expression level of VEGF in breast infiltrating ductal carcinoma was 44 cases while low expression was 32 cases.3 The relationship about clinical indicators between expression of IL-4,CD163, VEGF and breast cancerThe different density of CD163 in tumor stroma had related to lymphatic metastasis and vascular invasion. The difference had no statistically significant(P>0.05).The different density of CD163 surround the tumor stroma had related to clinical staging, lymphatic metastasis and vascular invasion. The increasing trend of density along with the increasing of clinical stage. The difference was statistically significant(P<0.05). But which had no significant correlation with age, tumor size, pathological stages and Luminal typing. The difference had no statistically significant(P>0.05).IL-4 expression had relevance with Luminal typing, lymphatic metastasis and vascular invasion. The higher the expression of IL-4 the higher rate of lymphatic metastasis and the higher the incidence of vascular tumor bolt. The high expression of IL-4 often occurs in patients with Luminal B. The difference was statistically significant(P<0.05). But which had no significant correlation with tumor size, age, pathological stages and clinical staging. The difference had no statistically significant(P>0.05).VEGF expression had relevance with Luminal typing, lymphatic metastasis and vascular invasion(P<0.05). The higher the expression of VEGF the higher rate of lymphatic metastasis and the incidence of vascular tumorbolt. And the high expression of VEGF more occurs in patients with Luminal B. But which had no significant correlation with tumor size, age, pathological stages, clinical staging. The difference had no statistically significant(P>0.05).4 The results of univariate regression showed that age, the expression of IL-4 and histological grading and patient survival time(OS) had no significant correlation(P>0.05). Tumor clinical staging, tumor size, Luminal typing,lymphatic metastasis, vascular invasion, the density of CD163 in tumor stroma and surround the tumor stroma, the expression of VEGF had significant effect on total survival period, which was statistically significant(P < 0.05).5 The results of multivariate regression showed that age, tumor size,histopathologic grading, the density of CD163 in tumor stroma and IL-4 had no significant effect on survival time of patients, which was no statistically significant(P>0.05). While other results such as clinical stage, Luminal typing, lymphatic metastasis, vascular invasion, the density of CD163 surround the tumor stroma and the expression of VEGF had significant effect on total survival period, which was statistically significant(P < 0.05).6 At the expiration of five years of follow-up, 5 year survival rate of 76 cases of breast infiltrating ductal carcinoma patients was 78.76%. The density of CD163 in tumor stroma, the expression IL-4 and prognosis have no obvious correlation. The higher density CD163 surround the tumor stroma is, the shorter survival time will be, which has statistical significance(Plogrank < 0.05).In breast infiltrating ductal carcinoma, the higher expression of VEGF is, the shorter survival time will be, which has statistical significance(Plogrank <0.05).Conclusion:1 The different density of CD163 in tumor stroma had linear positive correlation with lymphatic metastasis and vascular invasion. The different density of CD163 surround the tumor stroma had linear positive correlation with lymphatic metastasis and vascular invasion. The increasing trend of density along with the increasing of clinical stage. The expressionlevel of IL-4 and VEGF had positive correlation with Luminal typing,lymphatic metastasis and vascular invasion.2 The results of univariate regression showed that clinical staging, tumor size, Luminal typing, lymphatic metastasis, vascular invasion, the density of CD163 in tumor stroma and surround the tumor stroma, the expression of VEGF had influence on survival time.3 The results of multivariate regression showed that clinical stage,Luminal typing, lymphatic metastasis, vascular invasion, the density of CD163 surround the tumor stroma and the expression of VEGF had significant effect on total survival time.4 The survival time of breast infiltrating ductal cacinoma patients varied with the different density of CD163 surround the tumor stroma and the expression of VEGF. The higher density of CD163+ cell surround the tumor stroma is, the shorter survival time will be. And in breast infiltrating ductal carcinoma, the higher expression of VEGF is, the shorter the survival time will be. |
