Analysis On Risk Factors Of Ischemic Stroke Recurrence In 450 Patients

Objective: To research the risk factors of ischemic stroke recurrence and the recurrence of different TOAST subtypes,and to provide the basis for the prevention of ischemic stroke recurrence.Methods: Continuty to collect the first attack ischemic stroke cases from the Neurology Department of the Hebei Medical University between October 1,2014 and February 1,2015.Using self-designed questionnaires which including general demographic data,history of disease,test results and other information,Took incidence time of patients for follow-up starting point.Using telephone follow up for 1 year,took the recurrence of stroke,lost to follow-up,death as the ending point events.Follow-up contents included recurrence,drug taking.The factors including sex,age,hypertension,diabetes,coronary heart disease,smoking,drinking,body mass index,total cholesterol,low density lipoprotein,homocysteine,transient ischemic attack,carotid artery plaque,TOAST subtypes,compliance of antiplatelet drugs,compliance of statins drugs.Single factors analysis used Chisquare,Multi factors analysis used logistic regression.Results:1 Basic situation:Altogether collected 501 cases,lost to follow up 23 cases,eventually 408 cases included in the study,306 cases were men,144 cases were women,the median age of patients was 59.5 years old(16-81),there were 76 cases in recurrence group,and 374 cases in non recurrence group.In TOAST types,327(72.7%)cases were large artery atherosclerosis,92(20.4%)cases were small artery occlusion,18(4.0%)cases were cardioembolism,5(1.1%)cases were sroke of other detemined etiology,7(1.5%)were stroke of undetermined etiology.The recurrence rate of 3 month,6 month,1 year were 4.9%,8.4%,16.9%;2 Results of the logistic regression: Single factors and multi factors analysis showed that Hyperhomocysteinemia(OR=2.376,95% CI: 1.349-4.183),transient ischemic attack(OR=2.791,95% CI: 1.538-5.066),compliance of antiplatelet drugs(OR=1.941,95% CI: 1.116-3.374),cardioembolism(OR=1.941,95% CI: 1.116-3.374)were the risk factors of ischemic stroke recurrence.Conclusions: Ischemic stroke has a certain recurrence rate,Hyperhomocysteinemia,transient ischemic attack,compliance of antiplatelet drugs,large artery atherosclerosis were the independent risk factors of ischemic stroke recurrence.TOAST types can be used as predictor for recurrence of ischemic stroke,and cardioembolism was the independent risk factors of ischemic stroke recurrence.Age,sex,body mass index,hypertension,diabetes,coronary heart disease,smoking,drinking,total cholesterol,low density lipoprotein,compliance of statins drugs may have nothing to do with the recurrence of ischemic stroke in the early stage.
