Objective:In order to study the relationship between the expression of HPV E6/E7 mRNA and the level of HR-HPV DNA in diagnosis of the severity of cervical disease, the result of d HPV E6/E7 mRNA expression was compared with the result of HR-HPV DNA fluorescence PCR detection. The sensitivity and specificity of the two method in diagnosing cervical cancer was studied, and histology test was used as the gold standard to evaluate the effectiveness of HPV E6/E7 mRNA in diagnosing cervical cancer.Methods:The cervical cytological examination in 226 women with suspected cervical lesions were investigated by using the technology of TCT, and the specimens were divided into four groups:1) ASCUS (which makes no sense),2) LSIL,3) HSIL,4) SCC and ECA. With the aid of the optical electronic digital colposcopic examination and biopsy, the specimens were divided into cervicitis, CINI, CINII, CINIII, and cervical cancer. The kit for the detection of human papilloma virus nucleic acid (PCR-fluorescence) was used to detect HPV DNA. HPV E6/E7 mRNA expression was detected by bDNA technique from Cervical biopsy specimens, and its relationship was studied by pathological diagnosis.Results:1.The coincidence rate of the results of cytologic examination and the pathological biopsy rose significantly as the severity level of cervical lesions increased.2.The average age had no significant difference among the groups, from cervicitis to CIN Ⅲ (P>0.05). The average age of the group of cervical cancer was higher than the groups of CIN Ⅰ-Ⅲ (P<0.05), while there was no remarkably different between the groups of cervicitis (P=0.055>0.05).3.The detection rate of HR-HPV by HPV DNA was obviously higher than mRNA detection in the cases of cervicitis, CINI and cervical cancer, but the detection rate of mRNA was higher than DNA detection in the cases of CINII and CINIII, which was more consistent with pathological histology detection.4.The worse are the cervical diseases, the more are the consistence of the results of the test of HR-HPV DNA fluorescent PCR and HR-HPV E6/E7 mRNA5. E6/E7 mRNA detection is much more effective than DNA detection in terms of the specificity and positive predictive value, but both of sensitivity and negative predictive value are basically simillar.Conclusion:Detection of HPV E6/E7 mRNA plays a significant effect on monitoring and treatment of cervical disease. |
