Objective: To observe the lower lumbar spine(L3-5)pedicle narrowest pedicle height(Pedicle Outer Height,POH)pedicle width(Pedicle Outer Width,POW)difference(Pedicel Difference,PD),arch root coronal and sagittal plane angle between the long axis(coronal inclination,CI)variation,Discussion lumbar pedicle pedicle screw placement and shape matching.Methods :Select Medical University Affiliated Hospital of Southwest Spine Surgery July 1,2015-September 30,2015 in our hospital radiology undergoing total abdominal enhanced CT and lumbar CT + three-dimensional reconstruction of the crowd as the object to be measured,and ultimately selected 115 cases total of 690 vertebral pedicle,80 cases of male and 35 female,male patients,aged 18 to 78 years,mean 48.4 ± 14.2 years;145 178 cm average height 167.2 ± 6.1cm;weight 44 86 kg average 63.7 ± 10.0kg.Female group,aged 18 to 82 years,mean 48.20 ± 14.4 years;141 163 cm average height 154.4 ± 4.5cm;weight 38 71 kg average 54.3 ± 8.9kg.Are made of 64-slice spiral CT from L3-S1 continuous scanning,after scanning the original image is transmitted to the image processing ADW4.4 workstation,using MPR(multiplanar reconstruction,MPR)technique to obtain cross-sectional and coronal pedicle isthmus morphology observed and measured at its optimum level L3 to L5 pedicle narrowest POW,POH,calculate PD,measured CI value.Gender is divided into male and female groups;were divided into ≤60 years> 60 years of age groups;height divided ≤160cm,> 160 cm groups;BMI(Body Mass Index,BMI)points <24,≥24 two group.Vertebral pedicle was observed in each group and the difference between the height and width,the narrowest pedicle sagittal and coronal centerline axis angle(CI)variation;each vertebral pedicle corresponding to the height and width difference(PD)<0 percentage;pedicle width or height <percent of 7mm.Use Microsoft Office Excel 2003 and SPSS 17.0 software for statistical analysis,between the vertebral pedicle height and width difference,compared to the pedicle axis angle between the coronal plane were analyzed using pairwise comparison Q test and ANOVA(One-way analysis of variance,ANOVA).Results:The study collected a total of 690 115 cases of pedicle,male 80 cases of 480 pedicle 35 females 210 pedicle.Statistics showed that the spine on both sides of each corresponding POH,POW,PD and CI were no significant differences in the left and right from L3 to L5(P> 0.05).(1)lower lumbar spine(L3-5)corresponding to each spine POH were: L3(12.5 ± 1.4mm),L4(11.1±1.5mm),L5(10.4 ± 1.5mm),L3> L4> L5.POW were: L3(8.3 ±1.6mm),L4(10.1±1.7mm),L5(14.1±2.2mm),L3 <L4 <L5.PD were: L3(4.1±1.9mm),L4(0.9± 1.9mm),L5(-3.8±2.2mm),the maximum value L3: 9.1mm;Min at L5:-12.2mm,L3> L4> L5.CI were: L3(18.1±9.8°),L4(36.5± 9.6°),L5(43.3± 6.5°),maximum at L5: 66.2 °;minimum at L3 and L4: 0°,L3 <L4 < L5.(2)pedicle height and width difference(PD)percentage < 0mm from L3 to L5 as follows: 2.6,28.3,96.1.(3)from L3 to L5 vertebra all POH greater than 7mm,POWpercentage < 7mm from L3 to L5 were: Men: 10.3,1.7,0.Women: 14.2,2.7,0.Gender,age,height,body mass index packet,each male spine PD were less than women,men are larger than the average CI female,but there was no significant difference(P≥0.05);PD and CI gender,age,height,there was no significant correlation between body weight(P≥0.05),significant differences between the vertebrae,from L3 to L5 each corresponding spine PD,CI twenty-two by comparison,there are significant differences(P <0.05).Conclusion: 1,the lower lumbar pedicle PD from L3 to L5are: L3(4.1±1.9mm),L4(0.9±1.9mm),L5(-3.8 ± 2.2mm);L3> L4> L5,and PD were female than men,as a whole showed a larger variation POH next male and female POW of the lumbar spine.PD with age,height,weight,no significant correlation between vertebrae is an important factor of PD.2,the lower lumbar CI were: L3(18.1 ± 9.8 °),L4(36.5 ± 9.6 °),L5(43.3 ± 6.5°),L3 <L4 <L5,CI and gender,age,height,weight,no significant relevance,vertebrae is an important factor affecting the CI.3,pedicle height and width difference(PD)percentage<0mm gradually increases from L3 to L5 are: 2.6,28.3,96.1,when the difference is <0mm,select the pedicle screw diameter should be highly prevail.4.The study found that fromL3 to L5 vertebra all POH greater than 7mm,POW percentage <7mm fromL3 to L5 as follows: 12.3,2.2,0.POW ratio of less than 7mm,women significantly more than men. |
