Objective:To explore the effects of different training methods respiratory rehabilitation in patients with heart valve replacement surgery as randomed controlled trial research method, the study is to provide a reliable basis for clinical care, improve the patient’s prognosis, and promote early recovery of patients.Method:140cases of valvular heart disease as the object of this study, selecting from June2013to April2014at the Second Xiangya Hospital Cardiothoracic Surgery second, third wards in hospital, were randomly divided for the control and experimental groups as the use of socio-demographic data questionnaire.70patients in the control group received conventional training,70patients in the experimental group received comprehensive training in respiratory function on the basis of regular training, including abdominal breathing, pursed lips breathing, breathing exercises, multimedia education, effective pulmonary physical therapy. To observe postoperative ventilator time, the incidence of pulmonary complications, indwelling drainage tube time, hospital stay, time ECG, blood oxygen monitor data, Use T test, ANOVA, chi-square test methods for statistical analysis.Results:The all enrolled patients were successfully completed surgery and completed observation and analysis of the perioperative data. These parameters, between experimental and control groups, including the age, sex distribution, onset time before operation, lung condition, the type of surgery (mitral valve replacement surgery) and preoperative echocardiography examination, were no statistically significant (P>0.05) There were not postoperative death cases and hospitalization death cases in experimental and control groups. These parameters, between experimental and control groups, including respiratory ventilator functional training time, the incidence of pulmonary complications, indwelling drainage tube time, hospital stay, ECG time, blood oxygen detection were statistically significant (p<0.05), respectively, as follows:①ventilator time11.5±1.45(h) VS14.34±1.13(h);②incidence of pulmonary complications20%VS8.5%;③indwelling drainage tube time4.44±0.67(day) VS6.15±0.60(day);④hospital stay10.63±1.10(day) VS12.90±0.89(day);⑤ECG time36.51±1.61(h) VS41.51±1.94(h);⑥arterial partial pressure of oxygen90±5(mmHg) VS85±3.2(mmHg);⑦postoperative day blood oxygen detection98±1.8(h) VS93±2.8(h);⑧respiratory rate20.1±5.0(beats/min) VS26.1±7.2(beats/min);⑨maximal voluntary ventilation87.2±7.21(L/min) VS84.1±5.82(L/min)Conclusion:1. Through comprehensive respiratory function training, the indicators in the experimental group after the intervention had been greatly improved and enhanced compared with the before intervention, the mean heart rate, the mean respiratory rate had declined; and the mean oxygen saturation, the mean arterial oxygen partial pressure, the mean maximum voluntary ventilation were significantly improved.2. Through comprehensive respiratory function training, changes of the indicators in the experimental group was highly significantly than the control group, the difference of the indicators of before and after the experimental group was highly significantly than the control group.3. Comprehensive respiratory function training can reduce the incidence of postoperative pulmonary related complications of the patients who had been performed heart valve replacement surgery, reduce drainage tube indwelling time, improve postoperative subjective respiratory function obstruction degree, improve postoperative arterial oxygen partial pressure and oxygen saturation, reduce ECG time and time of hospitalization.
