Objective To analysis and explore the reasons of induced abortion, the situation and existing problems about post abortion care(PAC), the demand to PAC, then find ways to develop PAC.Methods: the quantitative and qualitative research method.1.In baseline survey encounter sampling method is used for the quantitative study, the objects of investigation come from induced abortion in an outpatient induced abortion room in a comprehensive hospital of Chongqing who will has an artificial abortion operation and voluntarily participate in the survey, with the method of questionnaire survey, the questionnaire will be recycle after finished. Use EPIdata3.1 and SPSS20.0 establishing database and analysis data.2. Qualitative approach was employed in this study with focus groups discussions and in-depth interviews in twenty objects; five of them are the doctor of induced abortion in an outpatient induced abortion room, the other is patients. At last, collects information carefully.Results1. Results of quantitative research:300 volunteers who were included in the study, 291 questionnaires were recovered; the qualified rate of questionnaires was 97%.1) The general condition of the research objects and their partner as follows: 76.97% of(224 of 291) them aged from 20 to 35 years old for woman, 35.74 %(104 of 291) of them were unemployed, educational level was high middle school or lower. 73.88%(215 of 291) of them were married and 58.42%(170 of 291) of them gave birth. 61.17%(178 of 291) of them aged from 20 to 35 years old for man, 58.42 %(170 of 291) of them were high middle school or lower education.2) The condition of abortion:Of the 291 objects, 199(68.38%) were contraception failed, and condoms failure the most(42.10%), just 37(20.33%) of them using condoms every time, and 92 did not use contraception, 67.39% of them who did not use contraception measures were in vitro ejaculation as a contraception method. There were 185(63.57%) objects have repeated abortion, 36.43 %(106 of 291) of all were first time. Condom was best used by major men 57.73%(168 of 291), the objects whose contraceptive method is condoms, only 20.33%(37of 182) of them used it every time when they have penetrative sex, 9.97%(29 of 182) of them were not adopting contraceptive methods. There were only 3.78% of women with intrauterine device.3) Scores of knowledge about contraception: the score of awareness of two groups were less than 6, accounting for 67.39% and 60.80%, and between the two groups differences with statistical significance(p=0.0001<0.05).Awareness by the higher the score, contraception into higher power.4) The causes of induced abortion: As the single factor analysis shows: the woman’s age, marriage status, the number of sexual partners, birth status and the man’s age, educational level, awareness of reproductive health and contraceptive knowledge are the causes of induced abortion, and statistical analysis showed that these causes between two groups were statistically significant.5) The demand for post abortion care: The main content about post abortion care were the care after abortion and contraceptive method, accounting for 76.63%(223/291) and 69.07%(201/291), the mostly educated way were by reading brochures77.32% and wall newspaper 80.07%; 51.4%(247 of 291) of them hope to got help from the medical staff; the effective and side effects about the contraception are two vital factors when they make the selection. The service attitude of the medical staff, the security of personal privacy, the environment and equipment of the hospital are the main factors when they chose a hospital, rather than waiting time and abortion expenses. It was worth noting that asked " if you want to get contraception knowledge", 57.04% choose "yes".2. Results of qualitative researchAll of the interviewees said: it is very necessary to carry out post abortion care and quality and efficient services can improve the level of reproductive health. Choosing an adaptive contraception methods and insisting were regarded as the most simple and effective way to birth control and repeated abortion control after induced abortion, at the same time strengthening the male companion education also is particularly important. The main contents about post abortion care were attention after abortion rather than introduction contraception method and timing. For now, repeat abortion rates still remain high, in addition to the object of the study to contraceptive knowledge awareness is low, and the executive force to doctor’s orders is not enough, the medical staff promotion ability was insufficient.ConclusionIt is widespread that the demand for birth control counseling service and post abortion care, but there is the insufficient service. In order to control of unwanted pregnancies and repeated abortion, also reduce or avoid from reproductive harm, hospital and community health service should strengthen the joint implementation to post abortion care, school reproductive health education.
