Objective:To discuss the relationship of serum tumor necrosis factor receptor 1(TNFR1), tumor necrosis factor receptor 2(TNFR2) with early diabetic nephropathy in type 2 diabetes patients. Methods:A total of 70 patients with type 2 diabetes and 35 healthy person were selected from December 2013 to November 2014 in the Fifth Hospital of Datong, divided into normal control group(n=35), simple diabetes group(n=35) and early diabetic nephropathy group(n=35), detailed record everyone`s sex, age, height, weight and course of diabetes, determined systolic blood pressure, diastolic blood pressure, triglyceride, total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol, glycosylated hemoglobin, serum creatinine, blood urea nitrogen(BUN) clinical indicators by group, and determined concentration of serum levels of TNFR1, TNFR2. Collected blood and urine of 24 hours, urinary albumin excretion rate(UAER) was determined by immune turbidity method, blood lipid and renal function was determined by enzymatic determination, HPLC method for determination of HbA1 c, ELISA method was used to determine TNFR1 and TNFR2, blood creatinine concentrations was used to calculate estimated glomerular filtration rate(e-GFR). Results:Compared with normal control group [TNFR1(302.5±60.0)pg/ml, TNFR2(62.1±9.8) pg/ml], simple diabetes group [TNFR1(515.1±78.1)pg/ml, TNFR2(178.5±38.9) pg/ml] and early diabetic nephropathy group [TNFR1(1066.8±205.6)pg/ml, TNFR2(279.6±37.1) pg/ml] all rised, and early diabetic nephropathy group was higher than simple diabetes group(P<0.05),TNFR1,TNFR2 level with age, course, HbA1 c levels were positively correlated, with HDL-C and e-GRF were negatively related, and the multiple stepwise regression analysis in e-GRF had significant effects on TNFR1,TNFR2 levels(P<0.001). Conclusion:(1) The level of TNFR1,TNFR2 in early diabetic nephropathy were elevated, TNFR1,TNFR2 may become new marks to evaluation early diabetic nephropathy;(2) TNFR1,TNFR2 level with age, course, HbA1 c levels were positively correlated;(3) TNFR1,TNFR2 level with HDL-C were negatively related.
