| Background Coronary atherosclerotic heart disease(coronary heart disease,CHD) has become one of the major diseases threatening human health.At present,a large number of studies have shown that dyslipidemia,especially high level of total cholesterol(TC)and low density lipoprotein-cholesterol(LDL-C),is one of the main risk factors of CHD.Strengthen treatment of dyslipidemia might be able to make obvious benefit in patients with CHD.The main mechanism of lipid regulating agents is to inhibit the synthesis of cholesterol or absorption.However, abnormal cholesterol metabolism can lead to an elevated TC and LDL-C.The main way of cholesterol metabolism in the body is to be converted into bile acid and be excreted through intestinal with bile.Some studies suggested serum level of total bile acid(TBA) associated with CHD,but there is little research which is to investigate the correlation between serum level of TBA and severity of coronary artery lesions in patients with CHD.If there is evidence to improve that serum level of TBA is correlated to the severity of coronary artery lesions in patients with CHD,we will find a new clinical indicators which may forecast the severity of coronary artery lesions,at the same time,we might obtain a new thinking of preventing and treating CHD.Objective To investigate the correlation between serum level of TBA and severity of coronary artery lesions in patients with CHD and to further explore clinical indicators which may assessment of the severity of coronary artery lesions,which provides newthinking for the prevention and treatment of coronary artery lesions.Methods A total of 120 hospitalized patients confirmed CHD by coronary angiography(CAG) were enrolled as CHD group and 60 cases excepted CHD by CAG were enrolled as control group.Serum level of TBA and results of CAG were collected.Gensini score was used to evaluate the severity of coronary artery lesions in the patients with CHD.Independent sample t test were used to compare the difference between two groups in serum level of TBA.Correlation between serum level of TBA and Gensini score were detected by Pearson correlation analysis.Using logistic regression analysis to analyze the multiple risk factors of CHD.Results Serum level of TBA was significantly higher in CHD group compared with that in control group(4.92±2.22)VS(3.01±1.89)umol/L,P <0.05;There was a positive correlation between Gensini score and serum level of TBA( r = 0.67, P < 0.05).Using TBA<4.0umol/L and TBA≥4.0umol/L as dichotomous variable, binary logistic regression analysis showed that TBA≥4.0umol/L was also a risk factor of CHD(OR:2.31, 95%CI:1.13~4.69,P <0.05) after adjusting the traditional cardiovascular risk factors.Conclusions Serum level of TBA is correlated to the severity of coronary artery lesions in patients with CHD.High serum level of TBA may be a risk factor of CHD.Serum level of TBA might be used as an index for prediction of the severity of coronary heart lesions.Intervention in bile acid metabolism may become a new way for the prevention and treatment of CHD. |
