| Objective:To explore the value of separate detection of serum tumor markers(TPA, AFP, CA242, CA50) in differentiating benign and malignant diseases of liver and biliary system. To find out the best tumor markers combination in differentiating the benign and malignant diseases of liver and biliary system, to explore whether there are obvious correlations between patient gender and tumor makers. Methods:Clinical data from January 2013 to January 2015 of patients from Shanxi Coal Hospital who got benign liver diseases(intrahepatic bile duct stones, liver cyst, common bile duct stone), malignant liver disease(primary HCC, intrahepatic bile duct carcinoma, extrahepatic bile duct carcinoma, pancreatic cancer) were collected, including patient gender, concentrations of serum tumor markers(TPA, AFP, CA242, CA50), and the indexes of liver function(ALT, AST, TBIL, DBIL, γ-GT, ALP) were analyzed. The values of AFP, TPA, CA50, CA242 were all expressed in(?x±s). T test and One-way analysis of variance were adopted to compare the groups of numerical, with P < 0.05, P < 0.001 for statistical significance. All data were analyzed by SPSS 13.0. Results:1.The serological levels of tumor markers AFP, CA50, CA242 and TPA in malignant disease group is significantly higher than the benign disease group.2. Statistical analysis showed that the six liver function indexes(ALT, AST, TBIL, DBIL, ALP, γ-GT) in seven groups of diseases have statistically significant difference.3. After the calculation of sensitivity, specificity and the Youden index for the combined detection of the four tumor makers, it is showed that combination of AFP and CA50 has better significance to differentiate benign and malignant liver disease; combination of TPA and CA50 has better significance in identification of benign and malignant biliary system diseases. The value of CA242 is not very obvious.4. Only AFP showed difference between genders, there was no difference between men and women in other three indicators. Conclusion:1.The serological levels of AFP, CA50, CA242, TPA significantly increased in malignant disease of the liver and biliary system.2. ALT, AST, TBIL, DBIL, ALP, γ-GT showed obvious difference between benign and malignant liver and biliary system diseases.3. It is recommended to combine AFP and CA50 in identification the benign and malignant liver diseases, while to combine TPA and CA50 in identification the benign and malignant biliary system diseases.4. No obvious correlation was found between gender and the tumor markers.5. Detection of serum tumor markers is an auxiliary diagnosis method in the clinical differential diagnosis. Doctors should utilize other information to make a final diagnosis. |
