| Objective: To compare the insulin resistance in patients with traditional open surgery and retroperitoneal laparoscopic radical nephrectomy in patients undergoing laparoscopic surgery,So as to analyze the insulin resistance of patients laparoscopic radical nephrectomy.Methods: 82 cases between 2012.10- 2015.6 in our hospital for treatment of kidney cancer patients for the study, based on the difference between surgery were divided into control group and observation group, the number of people in each group were 41 cases, the former is the traditional implementation renal open radical resection, which was implemented laparoscopic radical resection of kidney, ISI(insulin sensitivity index) compared two groups of patients, HOMAβ(insulin secretion index), FBG(fasting blood glucose), IRI(insulin resistance the index case) and FINS(insulin) and other indicators.Results: The two groups in BMI, age differences only comparable, no significant difference(P> 0.05); two groups of patients in the postoperative insulin secretion index and 24 h after contrast differences comparable only, no statistically significant difference(P> 0.05); preoperative and postoperative insulin sensitivity index 72 h, the index of insulin secretion and insulin resistance index difference was not significant(P> 0.05); blood loss between the two groups and the operation time difference is significant, and statistically significant(P <0.05); two groups of patients with insulin sensitivity index surgery, and postoperative 24 h contrast difference is significant, and statistically significant(P <0.05); two insulin sensitivity index in patients with insulin resistance index and 24 h after postoperative contrast, the difference was significant and statistically significant(P <0.05); two groups of patients before and after surgery the insulin resistance index(including postoperative 0h, 24 h and 72h) Compare and significant difference was statistically significant(P <0.05).Conclusion: The findings showed that insulin resistance index in two groups of patients after surgery showed an upward trend after 24 h, and 72 h after surgery are showing a decreasing trend, while in each of the above time period insulin sensitivity index is declining, group in the comparison shows that, in the presence of insulin resistance after surgery, and after the peak in 24 h, and later was a downward trend, and the faster rate of decline in the observation group than the control group. Based on this, we have to implement traditional open renal cancer patients after radical resection of renal and kidney laparoscopic radical resection of insulin resistance were significantly different, and the latter is lighter, but the latter is better insulin sensitivity. While the patient insulin resistance evaluation time, may also be insulin sensitivity index for reference, when the insulin secretion index difference was not significant, the traditional open renal radical resection and laparoscopic kidney radical resection phase larger than the wound. |
