Objective : To investigate the relationship and clinical significance between vascular endothelial function,blood pressure variability and carotid atherosclerosis in patients with primary hypertension.And to provide guidance for clinical treatment and prognosis of the patients with primary hypertension.Method:Selected in patients with essential hypertension(EH) as observation objects, the observation objects were divided into the vascular endothelial function normal group and the endothelial dysfunction group according to the results of FMD.Use the noninvasive portable blood pressure monitor( the type of DS- 250 made in Japan NISSEI company) to check the 24-hour blood pressure level and 24-hour blood pressure variation coefficient of the subjects. Use Toshiba S550 to exceed diagnose instrument testing carotid artery Toshiba S550 color Doppler ultrasound diagnostic instrument for the detection of carotid artery. Recorded and analyzed the relationship between the vascular endothelial function,blood pressure variability and carotid atherosclerosis in patients with primary hypertension.Results:The vascular endothelial dysfunction group(FMD: 4.58 ± 0.95) blood pressure variation coefficient(24 h SBP: 139.09 ± 10.45 mm Hg; 24 h DBP: 83.97 ± 8.26 mm Hg; n SBP: 133.72 ± 10.48 mm Hg; d SBP: 138.28 ± 11.52 mm Hg; d DBP: 86.22 ± 7.47 mm Hg; 24 h SCV: 12.26 ± 2.60; 24 h DCV: 17.18 ± 3.23; n SCV: 14.34 ± 2.67; n DCV: 17.04 ± 2.99) compared with the vascular endothelial function in normal group(FMD: 9.61 ± 1.06) blood pressure variation coefficient(24 HSBP: 121.00 ± 7.84 mm Hg; 24 HDBP: 78.81 ± 7.05 mm Hg; n SBP: 117.42 ± 7.56 mm Hg; d SBP: 133.45 ± 4.22 mm Hg; d DBP: 81.00 ±5.75 mm Hg; 24 HSCV: 10.56 ±2.18; 24 HDCV: 15.29 ±3.40; n SCV: 13.06 ± 2.73; n DCV: 15.33 ± 3.29), the difference was statistically significant(P < 0.05).The carotid intima-media thickness(1.17 ± 0.26 mm) of the vascular endothelial dysfunction group compared with the carotidintima-media thickness(0.84 ± 1.41 mm) of the vascular endothelial function of normal group,the difference was statistically significant(P < 0.05).Conclusion : The blood pressure variability index of the vascular endothelial dysfunction group is higher than the vascular endothelial function normal group, this suggest that endothelial function may be involved in the development of blood pressure variability. The carotid artery atherosclerosis of the vascular endothelial dysfunction group is particularly higher than the vascular endothelial function normal group, prompting the formation of atherosclerosis is closely related to vascular endothelial dysfunction.
