| Objective: To explore the clinical features and microsurgical treatment of pilocytic astrocytoma in the brain stem.Methods: the clinical features, treatment and prognosis of brain stem pilocytic astrocytoma patients admitted to the Department of Neurosurgery, Beijing Tiantan Hospital from 2013 to 2008 were analyzed retrospectively.Results: in this group, 27 cases were male and female, the proportion of male and female was 1.7:1, the median age was 21 years. Tumor location: 6 cases of brain, 5 cases of pons, 5 cases of cerebral junction of the pon, 11 cases of medulla. Clinical symptoms such as headache in 6 cases, 7 cases of vomiting, 5 cases of dizziness, diplopia in 9 cases, 3 cases of neck pain, ataxia in 6 cases, 11 cases of superficial sensory disturbance, movement disorders in 6 cases. The course of disease was 0.6-240 months, and the median course was 6 months. The head MRI showed T1 like on the other or low signal, such as T2 signal or high signal. Inhomogeneous enhancement in 22 cases. There were 18 cases with cystic components, and 9 cases were in parenchyma or accompanied with tumor necrosis. All patients underwent surgical treatment, tumors were totally removed in 13 cases, subtotal resection in 9 cases, partial resection in 5 cases, postoperative pathology in addition to a case of hair cell mucous astrocytoma were pilocytic astrocytoma. Among them, 4 cases in hospital radiotherapy, and 3 cases in hospital for chemotherapy. The average follow-up period was 35 months, 1 cases died, 1 cases recurred, 1 cases were operated again, and the KPS60-100 scores were followed up, and the median KPS90 score was.Conclusion: the growth of the brain stem pilocytic astrocytoma is slow, the symptom is changeable, the MRI has certain characteristic, the surgical treatment of the Department of neurosurgery is the ideal treatment method of this disease, and it can get good prognosis. |
