Intertrochanteric fracture is an international public health problem. Although it is more common in elderly patients, the optimal treatment of acute intertrochanteric fracture is still debated. The established surgical option includes internal fixation(extramedullary fiation, intramedullary fixation), hemiarthroplasy and THR. We analyze literatures which primarily focus on the clinical and functional outcomes of intertrochanteric fracture which had either undergone internal fixation, hemiarthroplasty or THR and to provide author a brief suggestion on selecting appropriate surgical option. We searched pubmed,medline and Cochrane database and collected articles which compared arthroplasty with internal fixation in the patients with intertrochanteric fracture. In concussion elderly patients with intertrochanteric fracture undergone arthroplasty shows superior result in term of post operative functional outcome and the rate of reoperations with lower rate of complication such as implant failure DVT, pneumonia and infection.
