objective:Through the analysis of PCI postoperative restenosis group and non restenosis, low metabolic rate is statistically significant and low lipid metabolism, reduce restenosis group, evaluation of statins metabolism related genes polymorphisms of PCI postoperative restenosis, statins metabolism related genes polymorphisms affecting the efficacy of lipid-lowering, statins individualized medication for guidance.method:1. In yanan university affiliated first hospital cardiovascular internal medicine in March 2014 to October 2015 and for ST elevation acute myocardial infarction, non st-elevation acute myocardial infarction, stable angina, unstable angina and line PCI surgery patients, according to the result of line 1 years after coronary CTA review the cases of PCI postoperative restenosis, 30 cases in the same period admission line of PCI and 1 year in coronary CTA review found no cases of PCI postoperative restenosis in 80 cases. All the cases were in the first line of PCI hospital started taking atorvastatin 20mg/d, preoperative oral 300 mg/d of aspirin, clopidogrel 600 mg/d, PCI postoperative application of low molecular heparin 5000 U once every 12 hours, application three days in a row. Atorvastatin after discharge piece doses are generally treatment dose(20 mg),every night before going to bed an oral, taking time for at least 1 year.2. ISR group and non ISR group general data comparison. General data of the two groups was hospitalized cases(including according to the results of the present study,thought of PCI postoperative restenosis related risk factors) no statistical difference, such as composition than men and women, smoking, blood pressure, blood sugar, blood fat,preoperative stenosis degree, implant stents number, diameter, etc.3. ISR group and non ISR group carry low metabolic phenotype. According to the testing results of statins metabolism related genes polymorphisms statistics in the two groups show the number of cases of low metabolism, statistical analysis, the restenosis group and non restenosis low metabolic rate is statistically significant.4. Low metabolism, normal metabolism and high metabolic group of blood lipid. 12 weeks after surgery and all patients review of blood lipids, comparing with lipid levels on admission for the first time, according to the results of genetic testing can be divided into low metabolism, normal metabolism and high metabolic groups, three groups are calculated separately, and the changes of blood fat, observation index of TC, TG, LDL-C, HDL- C, statistical analysis, observation of statins metabolism related genes polymorphisms of blood fat.results:1. ISR group and non ISR group general clinical data comparison. Ratio between the two groups, smoking history, history of hypertension, high blood sugar, blood vessel stenosis count, vascular stenosis site, quantity, stents were implanted stent diameter,length, degree of preoperative stenosis stent placement and laboratory tests: TC, TG,HDL- C, LDL- C, Lpa, TBIL, FIB and UA general clinical data no statistical difference.2. ISR group and non ISR group low metabolic phenotype carrying rate comparison.In ISR group characterized by low metabolism in 5 cases, low share of 16.67%, the metabolism in the ISR group characterized by low metabolism in 4 cases, metabolism of low share of 5.00%, low metabolic rate of two groups of statistical analysis, the results for X2 = 4.130, P = 0.042, with statistical significance(P < 0.05), can be thought of as low metabolism of PCI postoperative restenosis is influential.3. Low metabolism, normal metabolism and high group and the comparison of blood lipid metabolism. Three sets of changes of blood lipid, low metabolic group blood fat reducing levels of TC-1.42±0.53mmol/L 、 TG-0.34±0.22mmol/L 、 LDL-C-1.21±0.48mmol/L 、 HDL-C 0.19±0.14mmol/L, normal metabolic group blood fat reducing levels of TC-1.49±0.63mmol/L 、 TG-0.32±0.18mmol/L 、 LDL-C-1.40±0.50mmol/L、HDL-C 0.18±0.23mmol/L, high metabolic group blood fat reducing levels of TC-2.21±0.368mmol/L、TG-0.31±0.14mmol/L、LDL-C-1.75±0.46mmol/L、HDL-C 0.15±0.15mmol/L. The results show that the low metabolic group TC, LDL- C extent at least, the result is statistically significant(P < 0.05); Normal metabolism of TC and LDL- C has decreased slightly below the metabolome, but without statistical significance, low metabolic reactions to lipid-lowering medications can be thought of the worst.conclusion:1. ISR group and non ISR group difference was statistically significant lower metabolic rate, speculated that APo E gene polymorphism of PCI postoperative restenosis is influential.2. Low metabolism group, the normal metabolic group and high metabolic reactions to lipid-lowering drugs, low metabolic group of lipid-lowering effect is the worst,speculated that statins lipid-lowering efficacy is associated with APo E gene polymorphism.
