ObjectiveThis study was designed to understand the situation of reproductive health knowledge, attitude, premarital sexual permissiveness, contraception and impregnating partner unplanned about young male taking premarital examination in Liwan district, Guangzhou, and comprehensive evaluate and analyze its influential, in order to provide scientific basis for reproductive health research of unmarried young man. MethodsThe study is a cross-sectional investigation clustering sampled all first marriage’s male and their(quasi) spouse taking premarital examination at Liwan Hospital from January 2014 to February 2015 in Guangzhou, collecting information by questionnaire and examination data. The questionnaire includes reproductive health knowledge, premarital sex-related attitude, the use of contraception and the number of impregnating partner and so on. One hand, the questionnaire were distributed by doctors who were uniform trained strictly and the interviewer fill it unnamed in designated waiting room independently. The completed questionnaires were collected by the doctors checking the last item and were checked and numbered in time. On the other hand, the specially trained doctors using national premarital medical examination form and uniformed methods took the premarital medical examination. Data was entered in duplicate into Epi Data and after arrangement. All analyses were detected of logic and were performed by SAS 9.0 and SPSS 22.0. Statistical methods analyze and comprehensive evaluate the situation of reproductive health knowledge, sex-related attitude, premarital sexual permissiveness, the use of contraception and impregnating partner unplanned about young male taking premarital examination and its influential, which include chi-square test and Logistic regression model. Results 1. General information : Totally 721 pairs(quasi) couples were investigated. Age, education attainment, occupation and income was mainly constituted by age from 26 to 29(51.3%), college and above(83.6%), technical personnel(29.5%) and 3,001 to 5,000 yuan(39.3%), respectively. There were 28 pairs(3.9%) had been received marriage certificate. 2. The health situation of premarital man: The premarital man’s overweight and obesity rate was 24.7% and 7.9%, respectively. The prevalence of hypertension was 6.1%.The detective rate of genital disease was 7.3%, which included priapism, redundant prepuce, phimosis, orchidism, right side of the epididymis and spermatic cord varicose nodules. The number of them was 4, 11, 4, 9, 1 and 24.The positive rate of designated infective disease screening was 7.9%, which screening included hepatitis B surface antigen, syphilis, HIV and gonorrhea. The number of them was 53, 1, 1 and 2.The abnormal screening rate of genetic diseases was 23.2%, which screening included abnormal thalassemia, G6 PD and color vision. The number of them was 102, 34 and 32. 3. The situation of reproductive health knowledge among the male taking premarital examination: The total correct rate of premarital male’s reproductive health knowledge was 75.8%, which included four parts that is the physiological, contraception, sexually transmitted diseases and health. The correct rates of them were 51.6%, 81.6%, 90.3% and 77.6%, respectively. The were 90.3% reproductive health among premarital male qualified, which influencing factor included education attainment, income and their(quasi) spouse. The qualified rate among groups of college and above, income >5000 yuan and(quasi) spouse’s knowledge qualified was 93.2%, 94.0% and 92.5%, respectively. Moreover, the OR value of groups of college and above, income 3,000-5,000 yuan and 5,000 and above and(quasi) spouse’s knowledge qualified was 3.456, 2.185 and 2.372 and 2.530, respectively. 4. The premarital sex-related attitude of the male taking premarital examination: Premarital male’s attitudes towards “male’s premarital sexual permissiveness” and “female’s premarital sexual permissiveness” as follows: the conservative were 52(7.3%) and 66(9.3%), the neutral were 355(49.9%) and 396(52.6%), the open were 305(42.8%) and 270(38.1%), respectively. While to “premarital pregnancy”, the conservative was 419(58.7%), the neutral was 73(9.4%), the open was 222(31.1%).The number of open group of premarital sex-related attitude was 357(49.5%). STD knowledge and(quasi) spouse’s knowledge qualified were influencing the premarital sex-related attitude among premarital men. The open group of these two factors’ rate was higher, 50.5% and 60.2%. In addition, its OR value was 1.825 and 1.931, respectively. 5. The incidence of premarital sexual permissiveness of the male taking premarital examination: There were 650(90.2%) premarital man having premarital sexual permissiveness. Attitudes towards premarital sexual permissiveness and premarital pregnancy were the influential of premarital sexual permissiveness among premarital men. The incident rate of premarital sexual permissiveness of the open group of attitudes towards premarital sexual permissiveness and premarital pregnancy was higher, 94.0% and 95.0%, respectively. In addition, its OR value was 1.999 and 2.375. 6. The use of contraception of the male taking premarital examination: There were 245(37.7%) premarital man using contraception every time among 650 premarital man who having premarital sexual permissiveness. STD knowledge, the number of sexual partner and their(quasi) spouse’s reproductive health knowledge were influencing factors of the use of contraception among premarital men. The every time rate the use of contraception of STD knowledge qualified, 1 sexual partner and(quasi) spouse’s knowledge qualified was higher, 39.1%, 42.3%% and 41.7%, respectively. And its OR value was 2.498, 2.433 and 1.724, respectively. 7. The situation of the premarital male impregnating partner unplaned: Totally 98(13.6%) premarital man impregnating partner unplaned. Education attainment, occupation, contraception use, the number of sexual partner and(quasi) spouse’s knowledge were the influential of premarital man impregnating partner unplaned. The rate of occurrence group of high school/college and below, production/transport equipment operators, STD knowledge unqualified, using contraception usually, ≥2 sexual partners and(quasi) spouse’s knowledge unqualified was higher, 25.4%, 20.8%, 23.9%, 21.9%, 42.7% and 25.6%, respectively. The OR value of groups of high school/college and below, commercial/services and production/transport equipment operators, never/sometime/often use contraception, ≥2 sexual partners and(quasi) spouse’s knowledge unqualified was 2.231, 2.472 and 3.495, 3.721 and 4.401, 4.378 and 1.781, respectively. 8. The Comprehensive evolution of premarital sex-related behavior of the male taking premarital examination: In total, there was 417(57.8%) premarital man’s sex-related behavior in dangerous. Occupation, STD knowledge, sexual partner and(quasi) spouse’s knowledge were the influential of sex-related behavior. The dangerous rate of the unemployed/freelance, STD knowledge unqualified, ≥2 sexual partners and(quasi) spouse’s knowledge unqualified was higher, 74.0%, 78.3%, 84.1% and 72.5%, respectively. In addition, its OR value was 3.199, 2.423, 3.642 and 1.726, respectively. Conclusions 1. The male’s level of reproductive health knowledge, taking premarital examination, is higher. Its influential were education attainment, income and their(quasi) spouse’s knowledge. The main access of them is mass media, but the more still want to get more knowledge through professional channels. 2. The same of attitude about premarital sexual permissiveness between male and female among premarital men. However, they are still conservative towards premarital pregnancy. The influential of premarital man’s premarital sex-related attitude were STD knowledge and(quasi) spouse’s knowledge. 3. The incidence of premarital sexual permissiveness among the male taking premarital examination is higher. Moreover, the every time contraception is low, the incidence of impregnating partner before marriage is higher too. The influencing factors of sex-related behavior were occupation, STD knowledge, sexual partner and(quasi) spouse’s knowledge. These results indicated to build a comprehensive sex education system among unmarried young women and men, through enhancing perception about contraception among men for reduce the incidence of unplaned pregnancy. 4. The health status of male taking premarital examination is worse. They mainly suffered from designated infectious diseases and reproductive system diseases. Therefore, the health guidance needs to promote actively to improve healthy marriage, family life and birth quality.
