PURPOSE: To evaluate the reliability of High-Resolution MRI Technique in preoperative TNM staging in Patients with Rectal CarcinomaMETHODS:seventy and seven paitients with rectal carcinoma were diagnosed and treated consecutively in Department of Colorectal &Anal Surgery of Shanxi Province Tumour Hosipital during January 2014 and Jamuary 2015. seventy and seven paitients with rectal carcinoma meet the inclusion criteria. All of them underwent MRI of abdomen and pelvis and were staged in order to MRI beforoperation. We compared the MRI staging with the final histpathological results in order to evluate the reliability of MRI in preoperative staging(TNM) of rectal carcinoma.RESULTS: The sensitivity、specificity、positive likelihood ratio、negative likelihood ratio、positive predictive value、negative predictive value、area under curve o T2 staging tumors are 22.2%,93.6%,3.4,0.83,79.3%,92.1%,0.574 for p T3 staging tumors are84.8%,80.5%,2.38,0.18,88.9%,85.2%,0.822 for p T4 staging tumors are 80.0%、88.1%、6.72、0.22、82.1%、84.8%、0.840。The sensitivity、specificity、positive likelihood ratio、negative likelihood ratio、positive predictive value、negative predictive value、area under curve o N0 staging tumors are 22.2%,93.6%,3.4,0.83,79.3%,92.1%,0.574 for N1 staging tumors are 84.8%,80.5%,2.38,0.18,88.9%,85.2%,0.822 for N2 staging tumors are80.0%、88.1%、6.72、0.22、82.1%、84.8%、0.840。CONCLUSION:Preoperative high-resolution rectal MRI is accurate in pre-dicting tumor stage and CRM involvement, which are the main factors affecting the outcome of surgery.MRI rep-resents a precise diagnostic tool to select patients who may benefit from neo-adjuvant therapy and to avoid over-treatment in those patients who can proceed directly to surgery.
