ObjectiveTo assess the nutritional status detailly of ulcerative colitis patients before the 1st stage of IPAA operation and give preoperative nutritional support, to explore its effect on patients’postoperative rehabilitation, and analyse other relevant factors.MethodsThis research adopted the investigative and experimental research methods.The intervention group selected 40 UC patients diagnosed by medical history、 colonoscopy and pathological results, needing to undergo the 1st stage of IPAA operation, between January to December 2014, in general surgery department of one hospital.41 UC patients were selected as control group by referring to records, between January to December 2013, in the same department in hospital and accepting surgery treatment.Investigative research:The nutritional status of patients was detailed evaluated including body mass index, blood biochemical test results, human body surface measurement and REE, etc.Experimental research:On the basis of the evaluation results and the urgency of the disease,preoperative nutritional plan was formulated.Patients who were suffering acute stage was forbided eating and drinking,were given TPN treatment only planded by professional dietitian with energy (1060-1850 kcal/day).Within the stage of remission, patients complied with the guidance of medical staff to take liquid diet with enteral nutrition.the records of control was getted by researchers from the record room. After 1,3,10 day of operation,the nutritional results of of be two groups were contrasted.Social demographic data was collected by questionnaire.All data collected was processed by SPSS 17.0 statistical software. The statistical methods included K-S test, t test, χ 2 test, multiple linear regression analysis.Results1 Within the intervention group,21 cases suffered for body mass index, albumin, hemoglobin, prealbumin falling.Men and women grip strength all had different degrees of decline. 2 Nutrition metabolism results : The nutrition assessment score of 40 patientsin intervention group was low, resting energy expenditure was significantly higher than expected, and influenced by albumin and nutritional evaluation score greatly.3 Postoperative nutritional status and recovery:In the intervention group, hemoglobin and albumin increased more rapidly, subcutaneous fat increase gradually, gastrointestinal adverse reactions to reduce greatly, stitch and length of hospital stay were shorter, and so on.Conclusions1 Most of UC patients in surgical dpt,have severe disease, high incidence of malnutrition. Most of the them have a low level of hemoglobin, albumin, prealbumin and grip strength. Nutritional problems urgently needed to be solved in order to smoothly pass the perioperative period.2 Before operation, UC patients have a high metabolic state, which was greatly influenced by albumin, nutritional assessment scores.3 Although routine postoperative nutritional intervention can improve the nutritional status of patients, preoperative nutritional support’s effect is more apparent.
