Objectives:To investigate the relationship between liver volume and liver function,remnant liver volume and liver failure after hepatectomy in patients with primary hepatic cancer complicated with hepatitis B.Materials and methods:60 cases of primary hepatic cancer complicated with hepatitis B were selected in our hospital and underwent 256-slice CT whole liver perfusion scan,Including 30 cases of grade A,18 cases of grade B,12 cases of grade C.The images of all patients were introduced into EBW workstation to obtain the liver volume (excluding tumor volume),and per BSA (body surface area) liver volume.Compared to the above indicators with hepatic function Child-Pugh grade.In addition,14 cases of primary hepatic cancer patients underwent anatomical liver resection.The volume of resected specimen was measured by drainage method and CT respectively.Compared to the volume of the resected specimen obtained by the two kinds of methods.Postoperative patients were divided into normal liver function group and liver function failure group,compared to the remnant liver volume and per BSA (body surface area) remnant liver volume.Results:The liver volume and per BSA (body surface area) liver volume of the patients of primary hepatic cancer complicated with hepatitis B was decreased by Child-Pugh grade.There were all statistically significant differences between the two groups (P<0.05),And related to the clinical Child-Pugh classification,the Correlation coefficient is -0.681,-0.776.The volume of the resected specimen by the two kinds of methods no statistical differences (p>0.05).After the hepatectomy, the remnant liver volume of the liver function failure group was smaller than that of the normal group.The data of the two groups were statistically significant (P<0.05).Conclusions:(1)The liver volume and per BSA (body surface area) liver volume of the patients of primary hepatic cancer complicated with hepatitis B decreased with child-pugh grading, and was negatively correlated with child-pugh grading. So volume parameters can be used as a better indicator of liver function. (2)256-slice CT liver volume measurement can accurately response the living liver volume. (3)The remnant liver volume and remnant liver volume of body surface area is an important reference index to evaluate postoperative liver failure.
