| abstract:objective:this study means to investigate the differences between laparoscopic and open liver resection in right poster lobe, to give a preliminary conclusion on the effectiveness and safety of laparoscopic right poster lobe resection.methods:this study included 17 patients who underwent laparoscopic liver resection (among which, there were 5 cases which were converted to open surgery) and 17 who underwent open liver resection in the right poster lobe between January 1,2013, and December 31,2015 in sir run run shaw hospital. Their preoperative condition, operation time, blood loss and transfusion during operation, postoperative complication and postoperative hospital stay, expenses are evaluated.results:There were no significant differences between the LLR and OLR groups in preoperative condition. There were no significant differences between these two groups in blood loss, postoperative complication, length of postoperative hospital stay (p>0.05). The LLR group may have lower tranfusion rate than the OLR group. As for the liver function after operation, the LLR group have a significant lower ALT lever than OLR group (p=0.03). The LLR group may have a higher expense than the OLR group. There is no death within 90 days after operation, except for two default cases in each group. All margins for malignant cases are negative. Those underwent laparoscopic liver resection can be divided into two groups, one group was consisted with 5 cases who were converted to open surgery, the other group were those who were not converted. There was no difference between these two groups in preoperative condition. There was no difference in blood loss, post operative complications, blood transfusion rate, hospital expense. The length postoperative hospital stay of the converted group was longer than the other group (p<0.05).conclusion:LLR for right post lobe is non-inferior to OLR. compared with OLR, LLR is a relatively safe and effective operation. |
