Objective:To assess the respectability and prognosis of hilar cholangiocarcinoma by Bismuth-Corlette, TNM and Jarnagin-Blumgart. Meanwhile establishing a improved model foraccurate prognosis evaluation of hilar cholangiocarcinoma.Methods:Retrospective analysis of 154 cases of clinical and pathological data of hilar cholangiocarcinoma, according to Bismuth-Corlette, TNM, and Jarnagin-Blumgart, comparing surgical resection rate and prognosis. Modified Jarnagin-Blumgart staging system was used to estavlish for prognosis evaluation.Results:The resection rate of J-B T1 was 68.6%, which was higher than J-B T2 (44.8%, P= 0.007). The resection rate of J-B T2 was higher than J-B T3 (19.2%, P=0.025). By Bismuth-Corlette classification:The radical resection rate was 63.2%,57.6%,52.3%, 11.1%. By TNM staging system:26,30,21 and 2 patients were undergoneradical excision from type I to type IV. There was no difference between B-C and TNM in resection rate. The accuracy of improved J-B on survival time was 69.5%, which was higher than TNM and J-B.Conclusion:The J-B stage had more advantages than type B-C in predicting surgical resection, although the value of survival time was less than the TNM staging. The improved J-B was superior to the others in Prognosis.
