Objective:Lumbar disc herniation is defined as Intervertebral disc tissue localization displacement over the intervertebral space. After extensive research, this definition was proposed by the famous German scientist Schmorl in 1927 to 1931. But Dr. Barr and Dr. Mixter in The Massachusetts General Hospital published in August 1934 in the "new England Journal of medicine" Intervertebral disc rupture involving the spinal canal finally explained: Adjacent by herniated disc tissue compression of the spinal nerve root or spinal cord, clinical symptoms and signs and displays, is "lumbar disc herniation". They found that explains the issue of the lumber pain and sciatica which confused man for thousands of years, ushered in a new era of human understanding of the pathological changes of intervertebral disc. On the basis of these knowledge, Chinese medical workers combined with the development of medical basic research and related disciplines to develop a variety of treatment of lumbar disc herniation. It is well known that in lumbar intervertebral disc herniation with non surgery therapy, massage is one of effective methods treatment, but the accuracy of the method of operation determines the effectiveness of the treatment. Common method focus on treatment of pain points in the L1-L5 transverse process of the body surface or around the location, this may lead to a waste of time and unnecessary consumption of physical strength. In our study, a series of symptoms caused by lumbar disc herniation in normal adults will be studied in detail, to observe the therapeutic effect of different massage manipulations in the treatment of lumbar disc herniation, and to provide statistical basis for the clinical treatment of lumbar disc herniation with massage manipulation. Methods:Select 60 cases by the outpatient department of Guangdong traditional Chinese and Western medicine combined hospital and the Third Affiliated Hospital of Southern Medical University in the Department of rehabilitation medicine, diagnosed as cases of lumbar intervertebral disc herniation,60 cases were randomly divided into two groups. The treatment group (n=30) treated by Cha’s traction-punch press, routine treatment group (n=30) using conventional techniques for treatment, changes of pain severity, onset time, symptoms and signs of patients were observed. Before and after treatment of lumbocrural pain score (pain assessment scale) and evaluate its clinical effect. The spss20.0 statistics software to carry on the analysis, the statistical methods using t test, variance analysis, count data using x2 test and non parametric test, level data using Ridit analysis, α=0.05 level test, P> 0.05 said there was no significant difference, P<0.05 said the difference was statistically significant, P<0.01 said very significant difference. Results:Before treatment, the two groups of patients in gender, age, course of disease, symptoms, and other basic data were statistically different (P> 0.05), indicating no significant difference between the two groups.Through the statistical analysis showed that the five day of treatment, the pain symptom total score difference of conventional groups was statistically significant (P<0.05), no significant leg symptoms, symptoms, self-care ability and walking life symptom score score difference (P>0.05); the routine group cases was statistically significant at the five day of treatment of bone nerve signs, tenderness score difference (P<0.05), straight leg raising signs, signs, signs of each movement disorder total score have significant difference (P<0.01), no statistically significant differences in sensory disorder symptoms score (P>0.05); the treatment group were treated for five days when the pain, leg numbness symptoms, walking the symptoms, the individual symptoms total score have significant difference (P<0.01), no statistical significance of life and self-care ability score difference (P>0.05); the treatment group were treated for five days when the bone Nerve tenderness, straight leg raising signs, each individual signs total score have significant difference (P<0.01), statistically significant dyskinesia score difference (P<0.05), no statistically significant differences in sensory disorder symptoms score (P>0.05); routine group at the ten day of treatment, leg numbness, pain symptoms foot symptoms, life and self-care ability, individual symptoms total score have significant difference (P<0.01); routine group at the ten day of treatment of bone nerve tenderness, straight leg raising signs, signs, signs of each movement disorder total score have significant difference (P<0.01), statistically significant sensory disorder symptoms score the difference (P<0.05); the treatment group were treated for ten days when the pain, leg numbness symptoms, foot symptoms, individual symptoms score score score had significant difference (P<0.01), life And self-care ability score difference has statistical significance (P<0.05); treatment group were treatment for 10 days bone nerve tenderness signs, straight leg raising signs, signs of movement disorder, the individual signs total score have significant difference (P<0.01), feeling obstacle signs score difference had statistical significance (P<0.05).After treatment, the curative effect of the two groups was tested by P=0.027, and the Ridit was less than 0.05. Prove that there is a difference between the two groups of curative effect; total average 0.5000, each R value relation to treatment group (0.4178)< total average value (0.5000) and routine group 0.5822, total efficiency of conventional group (80.1%)< treatment group (93.3%). It is proved that the treatment group curative effect is better than that of routine group.Conelusion:1. In the treatment period, the treatment of lumbar disc herniation was directly proportional to the time.2. At the end of the first course of treatment, The treatment group in the legs hemp of symptoms and signs and symptoms, bone nerve tenderness on foot, Movement disorders is superior to the conventional group on signs of improvement.3. At the end of the second course of treatment, the curative effect in the treatment of Cha’s traction stamping method is equivalent to the conventional technique based. |
